Background: Rotavirus is globally the most common viral pathogen in childhood gastroenteritis. This study aimed to estimate the number of Turkish children suffering from early-childhood gastroenteritis by rotavirus by performing a meta-analysis. Methods: Meta-analysis following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines was performed. Following the guidelines, primary studies were found reporting the prevalence of rotavirus gastroenteritis in Turkey. We performed a computerized search of published studies in national and international databases from 1990 to 2018. We selected 38 out of 721 studies for our study. Meta-analysis was carried out using R statistical software. The Cochrane Q statistic was calculated to assess the heterogeneity of the study results. Heterogeneity among studies was evaluated using the I2 statistic. Effect-size estimate was reported with 95% confidence interval. Results: On the basis of 38 selected articles, 80,113 children up to five years of age were diagnosed with symptoms of acute gastroenteritis, of whom the stool samples of 13,651 children were positive for rotavirus. The pooled prevalence of rotavirus was 19% in children younger than five years of age with acute gastroenteritis. In terms of seasonal prevalence, the highest prevalence rate was found in winter. Conclusion: This study supports the major prevalence of early-childhood gastroenteritis by rotavirus among Turkish children. Therefore, the decision to adopt immunization programs to prevent rotavirus infection might be helpful in Turkey.

Introduction: Early detection of viral gastroenteritis is important because it causes hospitalizations in children and administration of antibiotics unnecessarily. Rotavirus and adenovirus are the most common viral pathogens in childhood gastroenteritis. The aim of this study was to investigate the rate of rotavirus and adenovirus in stool samples sent from children with acute gastroenteritis. Methods: The results of stool samples sent to the Tokat Gaziosmanpasa University Medical Faculty Research and Practice Hospital Microbiology Laboratory for the investigation of rotavirus and adenovirus antigens from children diagnosed with gastroenteritis between January 2017 and January 2018 were evaluated retrospectively. Rotavirus and adenovirus 40/41 antigens were detected by immunochromatography. Results: There were 1635 and 1608 patients' samples analyzed for rotavirus and adenovirus antigens, respectively. Rotavirus was detected in 133 (8.1%) patients, adenovirus in 30 (1.8%) and both agents in seven (0.4%) patients. Rotavirus antigen positivity was found to be significantly higher (p<0.05) in the patients aged between 13-24 months compared to other age groups. There was no significant difference between age groups in terms of adenovirus positivity (p>0.05). The majority of rotavirus gastroenteritis cases were seen in the winter (43.6%) and it was statistically significant (p<0.05). Discussion and Conclusion: Rapid antigen testing for viral gastroenteritis agents contributes to the planning of gastroenteritis treatment and decreases unnecessary antibiotic usage in children. It has been also concluded that it is important to consider the possibility of rotavirus gastroenteritis in children older than five years since 13.5% of patients were in this group.

Giriş: Bu araştırmada üçüncü basamak sağlık hizmeti veren hastanemizin acil servisine akut gastroenterit şikayetiyle başvuran hastalarda rotavirus ve adenovirus prevalansının belirlenmesi amaçlanmıştır. Yöntem: Çalışmaya akut gastroenterit şikayeti olan 1712 hasta alındı, çalışma retrospektif olarak hasta kayıtlarından yapıldı. Bulgular: Akut gastroenterit şikayetiyle 3 yıllık sürede acil servise başvuran 1712 hastadan dışkı örneği gönderildi. Bunların %17,5 'nde (n=301) rotavirus antijeni, %3,5'inde (n=61) adenovirus antijeni pozitif olarak saptandı. Pozitif olguların %56'sının (n=203) erkek, %44'unun (n=159) kadın olduğu gözlendi. Rotavirus antijeni 0-4 yaş grubunda diğer yaş gruplarında göre istatistiksel olarak anlamlı düzeyde yüksek bulundu. Sonuç: Akut gastroenterit, çocuklarda önemli bir sağlık problemidir. Özellikle 4 yaş altı çocuklarda rotavirus sıklığının fazla olduğu gözlendi ve bu yaş grubu hastalarda viral kültür yöntemlerinden ziyade rotavirus hızlı tanı testleri kullanılması erken tanı konmasına, böylelikle gereksiz antibiyotik kullanımının azalmasına yardımcı olabilir.
