Investigation into image quality and dose for different patient geometries with multiple cone‐beam CT systems

Abstract: Purpose:To provide quantitative and qualitative image quality metrics and imaging dose for modern Varian On-board Imager (OBI) (ver. 1.5) and Elekta X-ray Volume Imager (XVI) (ver. 4.5R) conebeam computed tomography (CBCT) systems in a clinical adaptive radiation therapy environment by accounting for varying patient thickness. Methods: Image quality measurements were acquired with Catphan 504 phantom (nominal diameter and with additional 10 cm thickness) for OBI and XVI systems and compared to planning CT (pCT… Show more

“…1,2,28 The patient separation (anterior/posterior and left/right), tumour volume and overall tumour size (gross tumour volume, at end exhale) were measured from the planning scans in the treatment planning system. Separation was taken to be the maximum separation on a CT slice containing the planning target volume.…”

“…According to Gardner et al.,4 the contrast‐to‐noise ratio of the pelvis compared to the spotlight protocol is significantly higher. Hence, the full rotation CBCT provides better soft tissue contrast compared to the half rotation protocol.…”

“…In the last years, many studies have been conducted about CBCT dose for different protocols 2, 3, 4, 5, 6, 7, 8, 9. Some of them evaluated only the CTDI values 3, 9.…”

“…These plans are designed to deliver a high radiation dose to the lesion and minimization of radiation dose outside the treatment region. 33,64 Radiotherapy planning requires the accurate reconstruction of attenuation coefficients of each patient for dose calculation, 31,32 which is difficult if the structures of interest lie in the same imaging plane as metal objects inside the patient's body. The combined kV and selective MV data may reconstruct on-board cone-beam CT images with fewer metal streak artifacts that allow better patient position verification.…”

“…Current radiotherapy devices often include detectors for both kV and full/truncated MV conebeam CT imaging. [21][22][23][24][25] The use of cone-beam CT increases patient setup accuracy, [26][27][28] and more recently with improvement in image quality has been considered for accurate tumor delineation, 29,30 and dose calculation, [31][32][33] which provide treatment verification and feedback information in adaptive radiation therapy (ART) techniques. 34,35 Fang et al proposed aggregated techniques for on-board CT reconstruction using a combination of full kV and MV beam projections to improve data-acquisition efficiency and image quality.…”
