Invasives Zervixkarzinom (pT2b-pT4a)

Abstract: At present, conventional T2-weighted SE images are superior to contrast-enhanced T1-weighted SE and pharmacokinetic MR images in depicting infiltration of the parametrium. However, suspected infiltration of the bladder and/or rectum (pT4a) is diagnosed more accurately on pharmacokinetic images than on conventional MR images.

“…In the vast majority of the cases, a good correlation with immunohistological assessment of tumor angiogenesis was observed [114]. In some situations CED-MRI analysis was a better predictor of disease outcome than histological evaluation of angiogenesis [115] or of circulating VEGF levels [116]. CED-CT is routinely used to measure cerebral blood flow, blood volume, mean transit time and vascular permeability in patients after ischemic brain lesions [117].…”

“…Use of a HTR sequence provides detailed information regarding the initial part of the time intensity curve including MSI, THR and EA. These semi-quantitative parameters are not only correlated with the number of microvessels in breast cancers (positive correlation with immunohistochemical CD31) (23,24) but also reflect parietal immaturity (20). We chose to use a 7.7 sec temporal resolution for our HTR-DCE sequence based on preliminary studies where the sensitivity of the dynamic sequence with a higher temporal resolution was decreased because of low spatial resolution (8).…”

“…Thus, it has been necessary to develop noninvasive surrogate markers of therapeutic response and efficacy measures for clinical trials involving antiangiogenic drugs [3,4]. Dynamic contrastenhanced magnetic resonance imaging (DCE-MRI) has emerged as a powerful noninvasive tool for serially assessing vascular dynamics within tumor tissue using parameters deduced from pharmacokinetic modeling [5][6][7][8][9][10][11]. This method exploits the abnormal hierarchical branching patterns and increased permeability of tumor vasculature [12 -14] through the influx, interstitial space extravasation, and washout of nonspecific lowmolecular-weight contrast agents.…”
