Intussusception of the Small Intestine Caused by a Primary Melanoma?

Abstract: Although the gastrointestinal tract is a fairly frequent site of melanoma metastases, reports of small bowel intussusception caused by melanoma are very rare. We report the case of a 77-year-old man who was admitted to our hospital with epigastric pain, melena and anaemia. After clinical examination, laboratory evaluation and radiological work-up the diagnosis of a jejunal intussusception was made. Exploratory laparoscopy revealed a large tumour arising from the jejunum, approximately 20 cm distal to the angle… Show more

“…Although a limited number of studies, intestinal obstruction secondary to intussusception is the most frequent clinical presentation of primary melanoma. 1,9,10,11,12 The presentation of our patient was ileocecal intussusception due to metastatic melanoma of the intestines.…”

“…Although small and large intestines typically contain no melanocytes, a few theories try to explain the origin of melanoma in the bowel. 4 , 7 The occurrence of primary colonic melanoma (PCM) – an extremely rare tumor of the gastrointestinal tract – could be explained by the fact that melanocytes have occasionally been found in the digestive and respiratory tracts. 7 Tumor regression could be another explanation for the occurrence of "metastatic melanoma in the colon" with unknown primary site.…”

Infection or other changes in the immune system can be associated with spontaneous regression of the melanomas' primary site.…”

“…Cutaneous melanoma arises from transformed melanocytes and can appear anywhere that melanocytes have migrated from during embryogenesis. The eyes [81], central nervous system [82], gastrointestinal tract [83,84], and even the gallbladder [85] have been reported as primary sites of the disease. More than 90% of melanomas are found on the skin and four percent are discovered as metastases without an identifiable primary site.…”
