Objective Intravesical prostatic protrusion (IPP) is a noninvasive parameter that can predict disease progression and development of complications in benign prostatic hyperplasia (BPH). This study was to determine the relationship between IPP and the presence of complications in BPH patients. Methods This was a cross‐sectional study of BPH patients at Enugu State University of Science and Technology Teaching Hospital, Enugu. Patients were assessed for acute urinary retention (AUR), chronic urinary retention (CUR), epididymoorchitis, hematuria, hernia, urinary tract infection (UTI), serum creatinine, and prostate‐specific antigen (PSA). They also had abdominal ultrasonography assessments for IPP, total prostate volume, bladder wall thickness (BWT), postvoid residual (PVR), hydronephrosis, bladder diverticulum, and urolithiasis using Sonoscape S11 with an abdominal‐probe frequency of 3.5 MHz. IPP was measured in millimeter and divided into <10 mm and ≥10 mm. Data were analyzed using SPSS version 21 and were subjected to 1‐way analysis of variance, chi‐square test, and Pearson correlation. The odds ratios of development of complications at an IPP cutoff of 10 mm were calculated. P < .05 was considered significant. Results A total of 118 patients with a mean age of 64.18 ± 10.96 years and a mean IPP of 14.29 ± 10.20 mm were included. Forty‐eight patients had IPP < 10 mm and seventy patients ≥10 mm. Patients with IPP ≥10 mm had significantly higher mean BWT, International Prostate Symptom Score (IPSS), PSA, and PVR (P ≤ .05) and significantly more AUR, CUR, hematuria, hydronephrosis, and UTI (P ≤ .01). Conclusion Patients with IPP ≥ 10 mm have a significantly higher incidence of some complications.

Objectives: To study the relationship between sonographically measured intravesical prostate protrusion and international prostate symptoms score (IPSS), as well as prostate volume, in men with benign prostatic hyperplasia at a single health facility. Materials and Methods: This was a cross-sectional observational study of one hundred men (age >40 years) diagnosed with benign prostatic hyperplasia. Their International Prostate Symptoms Score (IPSS) was assessed using the standardised IPSS instrument. Abdominal ultrasound was done to measure the intravesical prostatic protrusion (IPP), while prostate volume was estimated transabdominally and transrectally. Correlations between parameters were quantified with Spearman’s correlation test. P ≤ 0.05 was statistically significant. Results: The mean age was 62.84 ± 9.0 years (Range =42–79 years). The mean IPSS was 20.99 ± 6.42 (range of 5–30). Seventy-three percent of the men in this study had intravesical prostatic protrusion on ultrasound. The mean IPP was 13.0 ± 4.0 mm. Of the 73 men with IPP, 17 (23.3%), 29 (39.7%), and 27 (37%) had grade I, grade II and grade III IPP, respectively. The mean transabdominal prostate volume (TPVA) and transrectal prostate volume (TPVT) were 71 ± 14 ml and 69 ± 13 ml, respectively. IPP had a statistically significant positive correlation with all the other parameters. The strongest correlation (very high correlation) was with the TPVA (r=0.797, P < 0.0001), followed by a moderate correlation with the IPSS (r =0.513, P < 0.0001). TPVT, transition zone volume, transition zone index, presumed circle area ratio, and quality of life score yielded slightly weaker moderate correlation with IPP, while IPP correlated weakly with age. Conclusion: IPP correlated well with multiple clinical and sonographic parameters. It correlated better with TPVA than TPVT.

Background: Prostatic artery embolization (PAE) is an emerging minimal-invasive therapy of benign prostatic hyperplasia (BPH), able to reduce the prostatic volume (PVol) and the IPP. The presence of a true middle lobe (TML) is associated with bladder outlet obstruction (BOO), causing lower urinary tract symptoms (LUTS). In this study we investigate the effect of PAE in TML improvement in patients with LUTS. Methods: A retrospective analysis was done of 47 men treated with PAE from April 2015 to September 2021. The volume of the TML, IPP, and PUA were measured on MRI prior and 2 months after PAE. Successful devascularization of the TML was evaluated by contrast-enhanced MRI (ceMRI) 48 hours after therapy. Results: The TML was successfully embolised technically in 72%. After two months, the total volume of the prostate (PVol) was reduced by 25.8 ± 13.3% (from 72.1 ± 39.8 cc to 52.5 ± 27.9 cc; p < 0.000). Following a technically successful PAE of the TML, the TMLVol decreased by 32.1 ± 21.5% (from 10.6 ± 16.1 cc to 7.2 ± 13.1 cc; p < 0.000), and the IPP was reduced by 29.3 ± 15.5% (from 16.3 ± 7.4 mm to 11.9 ± 6.6 mm; p < 0.000). In contrast, after a technically incomplete devascularisation of the TML the TMLVol decreased by only 7.2 ± 17.7% (from 8.4 ± 9.3 cc to 7.5 ± 8.9 cc; p = 0.089), and the IPP was reduced by only 10.9 ± 8.8% (from 16.4 ± 7.3 mm to 14.6 ± 6.7 mm; p = 0.003). The currecture of the PUA after a successful and after an incomplete embolization of the TML was comparable with 11.6 ± 7.6 and 12.2 ± 9.4, respectively (in both p < 0.001). Conclusions: Our study firstly shows that PAE is able to reduce TML volume. Furthermore, PAE is able to reduce the IPP even if caused by a TML.
