Intraventricular and subarachnoid basal cisterns neurocysticercosis: a comparative study between traditional treatment versus neuroendoscopic surgery

Abstract: The authors postulate that MIFNES is a good alternative for the management of intraventricular and subarachnoid basal cisterns NCC because it allows removal of most of the parasites, rapid recovery of the patients, and removal and placement of shunt under direct vision when necessary. Traditional treatment is a second option where the MIFNES procedure is not available.

“…As far as surgical intervention is concerned, the application of neuronavigation techniques to remove basal subarachnoid cysts has been only rarely reported. 46,47 Also, excision of large Sylvian cysts even in the presence of other smaller cysts in a different location could greatly decrease the period of weeks or months of perilesional brain inflammation that follows anti-parasitic destruction of large parenchymal cysts. The advent of modern neuroimaging and effective anti-parasitic agents has greatly improved the management of NCC over the past 20 years.…”

“…About 30% of patients with hydrocephalus meet the criteria for this technique, which may prevent the need for a ventriculoperitoneal shunt, resulting in reduced costs and low incidence of late complications. 4,5,6,7,12,15,17 Nonetheless, because a ventricular endoscope is used through a single bur hole, additional resources to enable cutting, coagulation, and ablation of injuries are extremely relevant to reduce surgical time and hospital length of stay, and increase the safety of the procedure and the patient's postoperative comfort. 8,10,18 Increasing experience in several centers has proven that laser-assisted endoscopic procedures are useful options, because they are safe, effective, and advantageous in cases of distorted anatomy.…”
