Рассмотрен вопрос обеспечения седации в стоматологии и амбулаторной челюстно-лицевой хирургии� Подробно изложены и введены определения понятия седации, психоэмоционального комфорта и дискомфорта� Подробно освещена патофизиология психоэмоционального дискомфорта и периоперационного стресса� Согласно современным данным, переосмыслены цели, стоящие перед анестезиологом: дости-жение психоэмоционального торможения (как основы комфортного пребывания в стоматологическом кресле), а не угнетение сознания, как это было раньше� Подробно проанализированы осложнения, возникающие при проведении анестезии, отмечена их связь с возрастом, сопутствующей пато-логией, средством для анестезии, компетентностью врача� Представлена клиническая картина седации в зависимости от угнетения сознания, дыхания, гемодинамики (ADA и ASA, 2016)� Дано подробное описание состояния минимальной седации (анксиолизиса), умеренной седации (седация в сознании), глубокой седации и общей анестезии� Особое внимание уделено обеспечению безопасности пациента, связанной с поддержанием проходимости дыхательных путей и гемодинамики� Подробно представлены методы седации с позиции как современных, мировых данных, так и личного опыта авторов� Указаны методы введения препаратов: энтеральный, парентеральный, к которому можно отнести довольно популярный в настоящее время интраназальный метод введения� Отдельно обсуждены специфические проблемы седации, наличие двигательной активности, психоэмоционального возбуждения, навязчи-вого кашля� Описаны вероятные причины развития этих проблем, в частности связь психоэмоционального возбуждения с дисрегуляцией ГАМК-ергической и дофаминергической систем� Приведены современные данные об интраоперационном мониторинге как важном компоненте обеспечения безопасности пациента во время седации� Помимо Гарвардского стандарта, рекомендуется применение капнографии и мониторинга глубины седации при помощи биспектрального индекса� Ключевые слова: седация, психоэмоциональный комфорт, дискомфорт, стоматология и челюстно-лицевая хирургия, анксиолизис, монито-ринг, пропофол, мидазолам, дексмедетомидин, безопасность, общая анестезия The article describes sedation in dentistry and out-patient maxillofacial surgery� The definitions of sedation, psychoemotional comfort and discomfort are thoroughly described and introduced� A special emphasis is put on pathophysiology of psychoemotional discomfort and peri-operative stress� According to modern data, the objectives to be achieved by an anesthesiologist have been reviewed: provision of psychoemotional inhibition (as a basis of comfort staying in a dental chair), but not depression of consciousness as it used to be before� With much detail it analyzes complication occurring during anesthesia; their correlations with age, concurrent conditions, anesthetic agent and doctor's professional level are highlighted�The article presents a clinical picture of sedation depending on the depression of consciousness, respiration, and hemodynamics (ADA and ASA, 2016)� It depicts thoroughly the state of minimal sedatio...
