Intravascular Papillary Endothelial Hyperplasia of the Vocal Cord: A Case Report and Review of the Literature

Abstract: Objective: Rare disease Background: Intravascular papillary endothelial hyperplasia (IPEH), is a vascular tumor characterized by the proliferation of endothelial cells with papillary formations. It is a rare benign disease; therefore, it is important to exclude malignant vascular neoplasm in order to prevent misdiagnosis and inappropriate overtreatment. Case Report: Herein, we describe the case of a 51-year-old male who reported changes in his voice, and who was preoperatively diagnosed with vocal cord polyp a… Show more

“…Rossi et al emphasized the distinction between angiosarcoma and vascular malformations by the documented cases of angiosarcoma arising in hemangiomas and AVMs, highlighting the potential for sarcomatous transformation within pre-existing vascular lesions [ 18 ]. Also, the differentiation between intravascular papillary endothelial hyperplasia (IPEH) and malignant vascular neoplasms is crucial to prevent inappropriate overtreatment [ 26 ].…”

“…Some researchers believe that IPEH involves primary vascular endothelial cell hyperplasia followed by hemodynamic changes, which further lead to thrombosis [ 9 ]. Histopathological examination is crucial for the definite diagnosis of IPEH [ 5 ]. The typical histological features of IPEH include multiple papillary processes that are formed by noninvasive proliferating endothelial cells, which protrude into the vascular lumen [ 2 ].…”

“…IPEH is a benign disease that is mainly treated by the complete removal of the mass via surgery, and it has a good postoperative prognosis. At present, no cases of metastasis or recurrence have been reported [ 4 , 5 ]. Some scholars have proposed that radiotherapy can reduce the volume of residual tumors and may provide more lasting local control [ 15 ].…”

“…The diagnosis of PEH requires histologic examination as the radiologic appearances are nonspecific and may be similar to other vascular lesions including hemangiomas, Kaposi sarcoma, and angiosarcoma (17); thus these must be excluded from the differential diagnosis to confirm diagnosis as a clinically benign vascular entity. PEH is characterized by a proliferation of papillae lined by endothelial cells, closely associated with thrombus (18).…”
