Intravascular lymphomatosis presenting as adult respiratory distress syndrome

Abstract: An unusual case of intravascular lymphomatosis caused by small noncleaved, non-Burkitt's lymphoma, which presented with adult respiratory distress syndrome, is described. Extensive invasion of the small- and medium-size blood vessels of the lung, liver, spleen, kidneys, heart, esophagus, stomach, small and large intestines, bladder, and brain-but not the bone marrow or peripheral blood-is documented. The possible mechanism and the unusual features of this case are discussed in comparison with previously report… Show more

“…The other tumor can be either a hematopoietic or non-hematopoietic malignancy. Among hematopoietic tumors, NHLs are the most represented malignancies (about one third of cases) [51][52][53]. NHLs may occur before, concomitantly or after IVL diagnosis; the most represented histotypes are diffuse large Bcell lymphomas [54,55], but cases of small lymphocytic lymphoma [6], follicular lymphoma [51] or mucosa-associated lymphoid tissue-type lymphoma [15] have also been reported.…”

“…An (Table 3). Altered hepatic [53,61,62], renal or thyroid [63] functional tests are observed in near 15% of cases; these function tests are useful tools in staging of IVL; in fact, altered results are invariably associated with organ involvement by lymphoma cells.…”

“…Only a few cases have been reported in Korea, and these have involved nonspecific radiological findings, including diffuse interstitial infiltrates, pleural effusion, signs of pulmonary hypertension, and tumor-like consolidation in the lungs. [4][5][6][7][8] No PET images of pulmonary IVL have been reported. Interestingly, two recent reports suggest that PET is useful for evaluating IVL.…”

“…Most patients with pulmonary IVL show nonspecific radiological findings, including diffuse interstitial infiltrates, pleural effusion, signs of pulmonary hypertension, and tumor-like consolidation in the lungs. [4][5][6][7][8] The usefulness of positron emission tomography (PET) in diagnosing IVL has not been established, but Odawara et al and 2. Lung window of a coronal reformatted CT scan shows extensive ground-glass opacity in both lungs except for some areas (arrows).…”

“…Extensive involvement of brain, spinal cord, lungs, heart, spleen, kidneys, adrenal glands, omentum, skeletal muscle, bladder, gastrointestinal tract, pancreas, and lymphoid tissue is often observed during postmortem evaluation. 1,8,10 Antemortem diagnosis of human cases has been achieved following brain 1,4,8,11,12 or muscle [13][14][15] biopsy. Although it was not evident in this mare, neural manifestations are present in .80% of people affected by the condition 16,17 and, in human and small animal cases, have included ocular dysfunction, seizures, ataxia, circling, involuntary micturition and defecation, dementia, paresis, pain or altered pain sensation (hypesthesia/paresthesia/dysesthesia), and peripheral neuropathy.…”
