Purpose : To evaluate the influence of sperm characteristics on the outcome of infertility treatment using intrauterine insemination (IUI). Methods : Retrospective study of 431 infertility couples who underwent 1007 IUI treatment cycles from June 1999 to October 2002. Sperm parameters before and after preparation for IUI were evaluated and correlated with pregnancy outcome. Results : Clinical pregnancy occurred in 12% of cycles and 28% of patients. Initial sperm motility and processed forward progression were independently associated with pregnancy after IUI. The mean number of cycles per patient was 4.3. Although pregnancy rate per cycle did not differ from cycle to cycle, the cumulative pregnancy rate approached plateau after five cycles. Conclusions : Sperm motility is an independent factor influencing IUI-related pregnancy. A forward progression score of 3 to 4 in a processed specimen is necessary for IUI success. The number of IUI attempts per patient should be individualized depending upon the needs of patients.

SUMMARYObjective: To evaluate intrauterine insemination (IIU) procedures performed at the Infertility Service, Hospital Nacional Arzobispo Loayza, between January 1995 and December 1997. Material and methods: This is a retrospective and descriptive study. We include all infertile patients treated with IIU using husband's sperm (IE) or Giver's sperm (ID) during the period under study. Results: 98 patients (190 cycles) in IE group and 16 patients (33 cycles) in ID group were studied. We found significant association between the total number of motile sperm inseminated and pregnancy rates (PR). PR in IE group and ID group were 8,1% and 37.5% per patient and 4.7% and 18.1% per cycle, respectively. PR for cervical factor were 8.2% per patient and 4.1% per cycle. In cases of male factor PR were 13.3% and 11.5% per patient and cycle respectively. In IE group, PR increased in patients yonger than 36 years (9.8% per patient and 6.6% per cycle) and in patients without associated infertility factors (12.5% per patient and 6.6% per cycle). RESUMEN:Objetivo: Evaluar el procedimiento de Inseminación Intrauterina (IIU) en el consultorio de Infertilidad del Hospital Nacional Arzobispo Loayza entre enero de 1995 y diciembre de 1997. Materiales y métodos: Estudio retrospectivo, descriptivo. Se revisaron las historias clínicas de pacientes infértiles sometidas a IIU con semen del esposo (IE) o de donante (ID) que cumplieron con los criterios de inclusión. Resultados: Se incluyeron 98 pacientes (190 ciclos) en el grupo de IE y 16 pacientes (33 ciclos) sometidas a ID. No hubo diferencia significativa entre gestantes y no gestantes con respecto al tipo y tiempo de infertilidad, edad, factor asociado o uso de inductores de la ovulación. Se encontró asociación entre el número total de espermatozoides móviles inseminados y gestación (p<0.05). La tasa de embarazo fue de 8.1% / paciente y de 4.7% / ciclo en las pacientes con IE y de 37.5% /paciente y 18.1% /c en las pacientes sometidas a ID. La tasa de embarazos para factor cervical fue de 8.2% /paciente y de 4.1% /c. y de 13.3% /paciente y 11.5% /c. para el factor masculino. En las pacientes con IE de 35 años o menos la tasa de gestación fue de 9.8% /paciente y 6.6% /c. y para aquellas sin factor asociado fue de 12.5% /paciente y 6.6% /c. Conclusiones: Los embarazos se obtuvieron en relación a la concentración de espermatozoides siendo el método beneficioso en los pacientes con astenozoospermia.
