The dynamic of the placental transfer of digoxin has been studied in 12 pregnant dogs. 3H-digoxin was administered intravenously in 4 animals for 3 h. Specimens of blood were simultaneously collected from the mother and one fetus every 15 min and their radioactivity compared. Maternal and fetal ECG and arterial pressure were also recorded. The digoxin was given to 8 dogs for 3 different periods: (1) 3–4 days, (2) 8–10 days, (3) 12–14 days. Plasma concentration of digoxin of fetuses and mothers were calculated by the radio-immuno-assay technique. The study shows that digoxin enters the umbilical circulation soon after its appearance in the maternal blood; fetal plasma concentrations remain lower than the maternal for at least 4 days; however, the digitahzation effect can be observed on the fetal ECG within 3 h after administration of toxic doses of digoxin to the mother.

Fetal heart arrhythmias have been considered a sign of immaturity of the conducting system without clinical importance (Levkoff 1969), but certain arrhythmias, such as paroxysmal supraventricular tachycardia and bradycardia can have severe haemodynamic consequences (Silber & Durnin 1969;Schreiner et al 1978;Valerius et al 1978;Lingman et al 1980). Therefore, it is of interest to assess the haemodynamic effects of heart arrhythmias.…”

Summary. The effects of fetal heart arrhythmias were examined serially in two pregnancies by three non‐invasive methods: fetal ECG, fetal phonocardiography and ultrasonic measurement of fetal blood flow. In a case of supraventricular arrhythmia, there was evidence suggesting that the stroke volume varied with ventricular filling according to the Frank ‐ Starling law. In a case of total atrioventricular block the mean blood flow in the fetal descending aorta and in the umbilical vein was within the normal range. Blood flow velocity in the inferior vena cava of the fetus reflected atrial contractions. In the phonocardiogram, a phenomenon similar to‘bruit de canon’ was found. Both pregnancies had good outcomes and subsequent development of the infants was normal except for the persisting dysrhythmias. The two cases exemplify how fetal heart function can be assessed in utero.

Paroxysmal supraventricular tachycardia causing an intrauterine congestive heart failure was diagnosed in a fetus at 33 week's gestation. After maternal digitalization the tachycardias and the signs of heart failure disappears. Postpartum a Wolff-Parkinson-White syndrome was recognised by electrocardiography. The paroxysmal supraventricular tachycardias of the newborn was terminated abruptly by intravenous application of propafenon. Recurrences were prevented by the oral administration of propafenon.
