Intrathyroidal parathyroid carcinoma: a case report and literature review

Abstract: Parathyroid carcinoma is an uncommon endocrine malignancy comprising 0.5%–2% of patients with primary hyperparathyroidism. The probability of an intrathyroidal location is low (0.2%) and make preoperative suspicion and diagnosis challenging. Less than 20 cases of intrathyroidal parathyroid carcinoma have been reported. We introduce a case of intrathyroidal parathyroid carcinoma mimicking a suspicious thyroid nodule, and review the literature, with a focus on the role of adjuvant radiotherapy.

“…Thyroid nodules frequently co‐exist with IPAs 19 and several reports have highlighted concurrent IPA and nonmedullary thyroid carcinoma 61–63 . Moreover, there have also been several reports of intrathyroidal parathyroid carcinoma 64–67 . Other possible differentials include benign and metastatic lymph nodes, where they might be mistaken for partial IPAs if they are adjoining the thyroid.…”

“…Usually, there are two pairs of PTGs, but the incidence of supernumerary glands occurs on average in 15% of cases (range 3–25%), wherein their number can reach 12 pieces. Intrathyroidal location of PC is extremely rare, about 20 such cases have been described in the literature ( 5 , 6 ). Intrathyroid PC was more common in women ( n = 14) with a median age of diagnosis of 56 (36; 63) years.…”
