Intrapleural fibrinolysis and DNase versus video-assisted thoracic surgery (VATS) for the treatment of pleural empyema (FIVERVATS): protocol for a randomised, controlled trial – surgery as first-line treatment

Abstract: IntroductionPleural empyema is a frequent disease with a high morbidity and mortality. Current standard treatment includes antibiotics and thoracic ultrasound (TUS)-guided pigtail drainage. Simultaneously with drainage, an intrapleural fibrinolyticum can be given. A potential better alternative is surgery in terms of video-assisted thoracoscopic surgery (VATS) as first-line treatment. The aim of this study is to determine the difference in outcome in patients diagnosed with complex parapneumonic effusion (stag… Show more

“…Ved manglende bedring 48 timer efter påbegyndt behandling bør patienten konfereres med thoraxkirurgisk afdeling med henblik på kikkertbaseret operation for at opnå infektionskontrol. Det pågående danske FIVERVATS-multicenterstudie afklarer, om tidlig kirurgi inden for 48 timer er bedre end intrapleural terapi [24].…”

“…Some notable trials that are currently ongoing are the FIVERVATS study which is seeking to assess whether VATS or tube thoracotomy with intrapleural enzyme therapy is more effective in treating stage II and III complicated parapneumonic effusions/pleural infections [ 71 ]. Similarly, Chung et al seek to compare outcomes in the fibrinopurulent phase of pleural infection based on initial treatment with VATS or IR-guided chest tube with fibrinolytic therapy (results anticipated in 2024) [ 72 ].…”

“…However, there is heterogeneity in present treatment for phase 2 empyema. Treating with intrapleural brinolytic agents versus surgical intervention is a topic of hot debate, and current practice is not unanimous (14,15). On the other hand, surgical decortication is the consensus for phase 3 empyema treatment.…”
