Intraoral Transmission of Streptococcus mutans by a Dental Explorer

Abstract: A streptomycin-resistant strain of S. mutans was introduced into the mouth as adherent growth on an artificial fissure (AF). A second AF, which was initially sterile, was placed in a crown on the opposite side of the dentition. The labeled strain was not found in 8 initially-sterile AFs which were left in vivo for 2 to 6 days and were not examined with a dental explorer. The labeled strain was detected in 7 of 9 initially-sterile AFs which were probed with the dental explorer.

“…On the other hand, the use of a sharp probe along with the visual method does not appear to improve the diagnostic accuracy 16,21 . It may contaminate other sound sites 14 , damage the fissure 3 as well as facilitate the lesion's progression 29 . The main drawbacks of the conventional methods are that they still rely on the dentist's subjective interpretation, that it is difficult to evaluate a lesion's progression and that some clinicians decide on an unwarranted invasive intervention.…”

“…Moreover, those microbes can be transmitted via saliva flow 19) , periodontal probe 20) , explorer 21) and even byoral hygienic instruments 22) . Therefore, in the case of non-surgical periodontal treatment, access to the oral cavity as a whole might be more reasonable than access in the part.…”

“…As a result, the use of such instruments has been the subject of debate for years. Likewise, CP can transmit Streptococcus mutans from a contaminated fissure to a healthy fissure (Loesche et al, 1979). On the other hand, CP in combination with visual inspection (VI) does not improve the overall diagnostic performance of the exploration in application to caries of pits and fissures (Lussi, 1991;Lussi, 1993;McComb & Tam, 2001).…”

“…Enamel is a poor electrical conductor though caried enamel shows increased conductance versus intact enamel (Loesche et al, 1979). Demineralized enamel becomes more porous, fills with ion-containing fluid and minerals from saliva, and therefore exhibits increased electrical conductance (McComb & Tam, 2001).…”
