Waste audits in healthcare: A systematic review and description of best practices

Healthcare generates large amounts of waste, harming both environmental and human health. Waste audits are the standard method for measuring and characterizing waste. This is a systematic review of healthcare waste audits, describing their methods and informing more standardized auditing and reporting. Using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we searched MEDLINE, Embase, Inspec, Scopus and Web of Science Core Collection databases for published studies involving direct measurement of waste in medical facilities. We screened 2398 studies, identifying 156 studies for inclusion from 37 countries. Most were conducted to improve local waste sorting policies or practices, with fewer to inform policy development, increase waste diversion or reduce costs. Measurement was quantified mostly by weighing waste, with many also counting items or using interviews or surveys to compile data. Studies spanned single procedures, departments and hospitals, and multiple hospitals or health systems. Waste categories varied, with most including municipal solid waste or biohazardous waste, and others including sharps, recycling and other wastes. There were significant differences in methods and results between high- and low-income countries. The number of healthcare waste audits published has been increasing, with variable quality and general methodologic inconsistency. A greater emphasis on consistent performance and reporting standards would improve the quality, comparability and usefulness of healthcare waste audits.

The dramatic increase in medical waste has put a severe strain on sorting operations. Traditional manual order picking is extremely susceptible to infection spread among workers and picking errors, while automated medical waste sorting systems can handle large volumes of medical waste efficiently and reliably. This paper investigates the optimization problem in the automated medical waste sorting system by considering the operational flow of medical waste. For this purpose, a mixed-integer programming model is developed to optimize the assignment among medical waste, presorting stations, and AGVs. An effective variable neighborhood search based on dynamic programming algorithm is proposed, and extensive numerical experiments are conducted. It is found that the proposed algorithm can efficiently solve the optimization problem, and the sensitivity analysis gives recommendations for the speed setting of the conveyor.

La gestión de manejo de los residuos sólidos hospitalarios no ha sido conducido adecuadamente, originando problemas de precariedad del saneamiento ambiental, inadecuada segregación, mal manejo de los desechos que afecta a los trabajadores intrahospitalario y las múltiples enfermedades infectocontagiosas de riesgos físicos, químicos, microbiológicos y medio ambientales, en estos últimos años han tenido inconvenientes para el cumplimiento de la normatividad vigente en un enfoque de controlar y minimizar los desechos hospitalarios, así como reducir el efecto negativo a la población y el impacto ambiental que esto produce. El objetivo es analizar la mejora de la gestión y manejo de residuos sólidos hospitalarios evidenciado en una mejora o deficiencia, en cumplimiento de las normas técnicas del sistema de manejo de residuos sólidos hospitalarios. De manera que se empleó una revisión bibliográfica de artículos de científicos, permitiendo demostrar que existe aún deficiencia en la gestión de manejo de desechos hospitalarios no habiendo una mejora, representando un reto para el sistema de salud pública en América Latina y el Mundo, urgen intervenir en el sistema de gestión de desechos hospitalarios para aminorar el problema inadecuado de manejo de desechos hospitalarios que se convierte en un factor de alto riesgo sanitario y medio ambiente.
