Intraoperative performance and postoperative outcomes of endocapsular ring implantation in pediatric eyes

Abstract: Implantation of the Cionni CTR and single-piece AcrySof IOL in the capsular bag in pediatric eyes with subluxated lenses was safe and effective and led to a stable IOL with few significant complications.

“…However, cases in childhood are still commonly affected by these complications. The reported incidence of IOL malposition ranges from 4.7% to 33.3% [13][14][15][16] in children. Severe iridocapsular synechia, persistent postoperative uveal inflammation, pupillary dilation, a shallow anterior chamber, and asymmetrical fixation are potential consequences of pupillary capture in children [9][10][11].…”

“…The decentrations were asymptomatic and not progressive; therefore, surgical intervention was not required. Cionni et al 10 and Vasavada et al 12 did not report early decentration or other complications caused by underestimation of zonular weakness and implantation of a single-eyelet modified CTR. The angle of zonular dialysis or weakness in eyes with trauma may be more global than intraoperatively estimated.…”

“…The use of this modified CTR in different clinical conditions has been evaluated. [10][11][12][13] In this retrospective study, we report the clinical features and postoperative outcomes of patients with traumatic zonulopathy and cataract who had phacoemulsification with implantation of a posterior chamber IOL (PC IOL) and the modified CTR.…”

“…[1][2][3][4][5][6][7][8] However, the high level of difficulty in these cases may lead to intraoperative and postoperative complications such as crystalline lens dislocation, intraocular lens (IOL) decentration, and vitreous loss with consequential cystoid macular edema, uveitis, and retinal detachment. 6,9 Although small degrees of zonular weakness and small lens subluxations can be easily addressed with advanced surgical techniques, generally the greater the amount of lens subluxation, the more challenging and potentially disastrous the case may become. We present 2 cases of severe subluxated crystalline lenses that were treated with a primary anterior chamber IOL (AC IOL), yielding excellent postoperative results and no intraoperative complications.…”
