Intraoperative Hemodynamic Predictors of Mortality, Stroke, and Myocardial Infarction After Coronary Artery Bypass Surgery

Abstract: Intraoperative hemodynamic abnormalities, including pulmonary hypertension, hypotension during cardiopulmonary bypass, and postcardiopulmonary bypass pulmonary diastolic hypertension, were independently associated with mortality, stroke, and perioperative myocardial infarction over and above the effects of other preoperative risk factors.

“…Reich et al [22] suggested that intraoperative pulmonary artery pressure was independently associated with mortality and perioperative transmural myocardial infarction in patients undergoing CABG. High pulmonary artery pressure can be caused by left ventricular dysfunction after CPB.…”

“…Rose et al [7] could show in their study that there is an association between tachycardia occurring in the post-anaesthesia care unit and emergency surgical procedures as well as between tachycardia and procedures of longer duration. In a study on the influence of intra-operative cardiovascular events during cardiosurgical procedures on postoperative outcome, tachycardia was a predictor of peri-operative myocardial infarction and mortality [12]. The extent to which intra-operative tachycardia directly influences outcome or is indicative of problems in homeostasis, such as hypovolaemia, electrolyte and acid-base imbalances, could not be determined when based on the data analysis.…”

“…However, computerised anaesthesia record keeping has proved itself to be a reliable medium and superior to manual data entry [5,9,10,11]. Despite the increased data quality and other benefits of anaesthesia information-management systems (AIMS), there are only a few studies that investigate the influence of peri-operative cardiovascular events on morbidity and mortality [8,12].…”
