The authors report on the case of a patient evaluated for Valsalva maneuver-induced headache, dizziness, and ataxia. Neuroimaging revealed a Chiari I malformation without syringomyelia. A history of idiopathic hypertension was noted. After posterior fossa decompression, pathologically elevated blood pressure was absent, and at 24-month follow-up evaluation the patient remained normotensive. Although seemingly rare, this case illustrates that some patients with tonsillar ectopia may exhibit elevated blood pressure. Clinicians should consider hindbrain herniation a rare cause in idiopathic hypertension.

Twelve patients with Chiari II malformation developed laryngeal stridor and respiratory distress. This may develop when there is acute downward or upward displacement of the malformed brain stem secondary to increased or decreased intracranial pressure. Rapidly progressive hydrocephalus and compression of the lower portion of the IV ventricle are more important factors than the size of the head at birth. For the treatment of these symptoms relief of the increase in intracranial pressure, relief of the pressure difference between intracranial and intraspinal compartments, and selection of a shunt system with a proper flow rate are important. Posterior fossa decompression did not show a favorable effect.

Abstract: This is a report of 2 infants with Arnold-Chiari malformation who showed various degrees of cardiovascular instability prior to and during surgical decompression of the posterior fossa which was abruptly terminated with successful decompression.
