Objective. To compare the efficacy of subcutaneous extralesional triamcinolone acetonide injection versus conservative treatment for chalazion. Design. Randomised controlled trial. Setting. Eye clinics of two regional hospitals in Hong Kong. Patients. Patients over 18 years old presenting with primary chalazion were randomised into two groups. In group 1, 12 patients were treated with lid hygiene, warm compresses, and chloramphenicol 1% ointment 4 times a day. In group 2, 16 patients were treated with 0.3 mL triamcinolone acetonide (10 mg/mL) injection to the subcutaneous tissue extralesionally via the percutaneous route. Exclusion criteria were: acutely infected chalazion with preseptal cellulitis, recurrent chalazion, small chalazion (≤2 mm), and prior treatment to chalazion. Main outcome measures. Size of chalazion, recurrence of chalazion, intraocular pressure, and complications from treatment, including skin pigmentary change or atrophy and pyogenic granuloma.Results. There was a clinically and statistically significant difference between the success rates in group 1 (58.3%) and group 2 (93.8%). In group 1, the mean prior duration of chalazion before treatment was significantly shorter in success cases than in failed cases. One patient with multiple chalazia in group 2 developed hypopigmentary skin changes at one treatment site. Conclusion. Subcutaneous extralesional triamcinolone acetonide injection was more effective than conservative treatment for chalazion.

The therapeutic use of corticosteroids has long been associated with ocular side effects such as the development of cataracts, glaucoma, and retinal and choroidal emboli. This study was conducted to assess ocular disorders, which may occur in workers engaged in manufacturing corticosteroids in the pharmaceutical industry. The study population included 58 workers involved in the manufacturing of corticosteroid preparations. They were subdivided according to duration of exposure into 2 groups: Gr. I of <15 years duration of employment and Gr. II of 15 + years duration of employment. Methods included answering a structured questionnaire enquiring about occupational and medical histories, full ocular and clinical examination, and measurement of certain biochemical parameters, such as plasma cortisol, serum total cholesterol, serum triglycerides, fasting blood sugar and serum calcium. Results showed that 25 cataracts were diagnosed in the studied group with a rate of 21.55 per one hundred eyes. Of Gr.II, 43.75% of eyes had cataracts compared to 5.88% of Gr.I. Odds ratio to develop cataracts was 12.44 for Gr. II compared to Gr.I. Ocular hypertension and open-angle glaucoma were present in 25.00% of the eyes of the studied groups with odds of 5.83 times more when being employed for 15 or more years. Significant +ve correlation was found between intraocular pressure and relevant biochemical parameters. In the present study, a clear relationship was detected between the length of the period of exposure to corticosteroids as environmental pollutants and the development of ocular medical disorders such as cataracts, ocular hypertension and glaucoma. Further research work is needed to study the effects of exposure to corticosteroids as pollutants in the work environment, gaining access into body via unusual routes of entry, and for periods of time not reported in the therapeutic literature.

Диагноз «синусит» основывается на сочетании клинических данных с осмотром, данными инструментальных исследований. На вирусные ОРС приходится свыше 90% случаев заболевания. Бактериальная этиология синусита имеет место примерно в 55-65% случаев. Целью лечения острого РС является ликвидация очага инфекции, восстановление пассажа в области остиомеатального комплекса, налаживание дренажной функции, уменьшение воспаления и облегчение симптомов, таких как боль и заложенность носа. Основными направлениями лечения являются антибактериальная терапия, ирригация, противовоспалительная терапия. Топические кортикостероиды -препараты первой линии, наряду с ирригацией. Они могут использоваться как в качестве монотерапии, так и в дополнение к антибактериальной терапии. При легких формах ОРС мометазон в дозе 400 мкг/сут оказался значительно более эффективным, чем плацебо и амоксициллин. А добавление топических кортикостероидов (МФ) к стандартному курсу системной антибиотикотерапии (амоксициллин/клавуланат, цефуроксим) значительно быстрее по сравнению с плацебо облегчало симптомы острого РС и ускоряло выздоровление. Интраназальные стероиды помогают в лечении дневного кашля. При сравнении с лечением другими ИГКС мометазон обладает большей эффективностью, безо-пасностью, лучшим разрешением симптомов, меньшей стоимостью лечения. The sinusitis diagnosis is based on a combination of clinical data with examination, data of instrumental studies. Viral ARS accounts for more than 90 per cent of cases. The bacterial etiology of sinusitis occurs in around 55-65% of cases. The purpose of the treatment of the acute RS is to eliminate the focus of infection, restore the ostiomeatal complex, regulate a drainage function, reduce inflammation and alleviate symptoms such as pain and nasal congestion. The main areas of treatment are antibacterial therapy, irrigation, anti-inflammatory therapy. Topical corticosteroids -first line drugs, along with irrigation. They can be used both as monotherapy as well as in addition to antibacterial therapy. In the light forms of ARS mometazone at a dosage of 400 μg per day was much more efficient than placebo and amoxicillin. And the addition of topical corticosteroids (MF) to the standard course of system antibiotic therapy (amoxicillin/clavulanate, cefuroxime) was much faster than the placebo...
