Intraocular pressure and haemodynamic responses to insertion of the i-gel, laryngeal mask airway or endotracheal tube

Abstract: Insertion of the i-gel device provides better stability of IOP and the haemodynamic system compared with insertion of an endotracheal tube or LMA in patients undergoing elective non-ophthalmic surgery.

“…The haemodynamic response to insertion of i-gel was significantly less than that for endotracheal intubation (2,19) and is a reflection of an increase in sympathoadrenal activity due to oropharyngeal and laryngotracheal stimulation (20). I-gel has a soft gel thermoplastic elastomer cuff and may prevent stress stimulation.…”

“…Mean blood pressure [ Ismail et al, [28] measured Intraocular pressure (IOP), Haemodynamic responses in 60 patients divided into three groups receiving LMA, i-gel, Endotracheal tube and they came into a conclusion that i-gel insertion provides better stability of IOP and haemodynamic system when compared with LMA or E.T tube insertion. Our result was also similar with that of Ismail et al, Placement of any supraglottic airway device is expected to be associated with changes in HR, SBP, DBP and MABP possibly due to stimulation of receptors on the wall of the pharynx, resulting in a reflex sympathetic response.…”

“…Use of LMA and tracheal tube resulted in signifi cant increase in intraocular pressure and hemodynamics compared to I-gel 7 . Present study also revealed that there is increase in heart rate and blood pressure compared to baseline and intergroup comparison also resulted in maximum response with tracheal tube and least with I-gel use.…”
