Intraocular lens optic capture

Abstract: A continuous curvilinear capsulorhexis (CCC) provides a tear-resistant opening that allows use of a technique of capturing the intraocular lens (IOL) optic through the capsulorhexis opening when the opening is at least 1.0 to 2.0 mm smaller than the optic diameter. The technique provides stability and long-term centration of the IOL and prevents vitreous from extending anterior to the IOL. This paper reviews the current variations of optic capture including (1) haptics in the sulcus and IOL optic capture throu… Show more

“…In case 1, we achieved stabilization of IOL by fi xating it to sclera whereas in case 2, it was achieved by capturing the optic through an anterior capsule opening with haptics lying in the sulcus. The second patient had a less severe degree of megalophthalos in comparison to the fi rst patient and therefore IOL capture technique (as described by Gimbel et al 5 ) was adopted. In summary, these cases highlight the spectrum of challenges and their successful management while performing phacoemulsifi cation in patients with anterior megalophthalmos.…”

“…This is possible even if an anterior capsule rip is present, but the maneuver will have to be gently executed and caution exercised to ensure the haptics are placed away from the anterior capsule tear at the end of surgery. Anterior capture 16 (for the IOL in the bag, facilitated by using an IOL forceps such as the Kelman-McPherson forceps to grasp and lift the optic anterior to the CCC) or posterior capture A (for the IOL in the sulcus, using the Sinskey hook to push the optic through the CCC) is an important step that seals off the vitreous cavity and provides a platform over which phacoemulsification of residual fragments can be safely performed. Anterior optic capture can be reversed at the end of surgery if necessary.…”
