Intraneural Synovial Sarcoma of the Median Nerve

Abstract: Intraneural synovial sarcomas are extremely rare in the main nerve trunks of the upper limb. We report on a 16-year-old youth who presented with a painless mass on the flexor aspect of the wrist with the clinical appearance of a ganglion. At operation there was a tumour of the median nerve that was shown on histology to be an intraneural synovial sarcoma.

“…19 To the best of our knowledge, there have only been five cases of intraneural synovial sarcoma (INSS) reported in the literature. [20][21][22][23][24] Three of these cases were seen in teenagers (all 16 years old), while one occurred in a 23-year-old female patient and one occurred in a 43-year-old man. All patients presented with a subcutaneous soft-tissue mass involving major nerve trunks (the popliteal nerve, 23 common digital nerve, 20 median nerve 22 and radial nerve 24 ) often near body surfaces 21 with a frequent history of a previous trauma.…”

“…Intraneural synovial sarcomas are extremely rare in the main nerve trunks of the upper limb [4,7,26,32]. They were originally thought to originate from synovial membrane; however, less than 5% are microscopically in continuity with the synovium [9].…”

“…They may occur at any age but are more prevalent in young adults, and the extremities are the commonest site. In the upper limb, they can be confused clinically with a ganglion [4,10].…”
