Intractable Constipation in the Elderly

Abstract: Chronic constipation is a common gastrointestinal disorder disproportionately affecting the elderly. Immobility, polypharmacy, and physiologic changes contribute to its increased prevalence in this population. Unidentified and undertreated constipation leads to a significant negative impact on quality of life and an increase in healthcare spending. Careful physical examination and exploration of the clinical history can unmask primary and secondary forms of constipation, guiding diagnostic and therapeutic cons… Show more

“…In addition to ONS and SAB, laxatives and paracetamol were the other commonly introduced drugs during hospitalization, which is consistent with the fact that malnutrition [ 18 ], pain [ 21 ] and constipation [ 22 ] are major problems in the elderly. Except for ONS, which was not analyzed, our results are similar to those of a French study that assessed drug modifications among geriatric inpatients [ 8 ].…”

“…GI motility disorders, in particular, are prevalent amongst the elderly, and cause significant physical, emotional, and financial burdens due to chronic constipation, fecal incontinence, even GI tumors. Among them, the incidence of constipation is striking, about 20% of old people experience chronic constipation, which results in failure to absorb nutrients and eliminate harmful metabolites, as well as increasing incidence of infection, and so being involved in the etiology of GI diseases [1,2]. It is believed that the deeper understanding on the pathological proceeding of GI motility disorders will provide a special “window of opportunity” for the early intervention and delay disease progression in the elderly.…”

“…Moreover, this finding derives from a chi-square test and constipation is not exclusively a medication-related problem in the ID population [ 37 ]. On the other hand, the finding that the use of laxatives in our sample was significantly higher in people treated with antipsychotics and antiepileptics and in (psychotropic) polypharmacy (2+ psychotropics) treatment suggests that medications with or without AC activity plays an important role in chronic constipation in older adults both with [ 12 , 30 ] and without ID [ 37 , 38 , 39 ].…”
