Intracranial Melanotic Schwannomas

Abstract: Radical surgical resection is considered the treatment of choice for MS, but treatment guidelines still do not exist. Radiotherapy seems to play an important role in reducing the risk of recurrence in the case of subtotal tumor resection. Despite the reported encouraging results, only anecdotal data are available in the pertinent literature. Future studies should focus on the role of radiotherapy as adjuvant treatment when radical surgical excision cannot be achieved.

“…The imaging findings of a primary intraspinal melanocytoma are characterized using short T1 and T2 signals (72%). 12 , 13 MRI in our patient showed T1 hypointensity and a short T2 signal in the vertebral bodies of C2 to C4. These results were consistent with an MRI diagnosis of MS.…”

“…Radiotherapy should be considered in cases of tumor recurrence, metastasis, histologic criteria of malignancy, or incomplete surgical resection. 3 , 12 Thus, even when adjuvant radiotherapy is administered to avoid relapse, close observation should be continued. Following the operation and adjuvant radiotherapy in the present case, the patient’s limb numbness and weakness were alleviated.…”

“…Adjuvant radiotherapy remains controversial. Watson et al [1] suggested that, without histologic signs of malignancy, adjuvant treatment should not be performed and only strict radiologic and clinical follow-up is required. In our case, the tumor only presented a focally high Ki-67 index of 18%.…”

“…Our patient underwent a macroscopically complete resection. Vallat-Decouvelaere and Zhang [16] observed a lower rate of recurrence and metastases in patients with MS treated with adjuvant radiotherapy. According to these authors, radiotherapy should be considered in case of histologic criteria of malignancy, incomplete surgical resection, tumor recurrence, or metastases.…”
