Intracranial Aneurysms and Heredity

Abstract: The occurrence of intracranial aneurysms (IAs) in the families of 579 consecutive patients with subarachnoid hemorrhage (SAH), of whom 485 had verified IAs, was studied retrospectively. IAs occurred in the families of 6.7% of the IA patients, but only 0.4% of their siblings had IAs. However, there were differences between the familial and nonfamilial IA patients, indicating that the familial patients are a specific small subpopulation of IA patients. The familial patients were younger, often had multiple aneur… Show more

“…10 - 13 One recent study concluded that 7% of all intracranial aneurysms may be familial. 11 The familial occurrence of spontaneous extracranial carotid artery dissection has been described in two families seen in our institution. 14 Ehlers-Danlos syndrome type IV and Marfan's syndrome are heritable connective tissue diseases associated with intracranial aneurysms as well as cervical artery dissection.…”

“…Thus, a 2-to 6-fold increased risk of SAH has been reported among first-degree relatives of SAH index patients. [5][6][7][8][9][10] The largest documented pedigree included 6 clinically manifest individuals and displayed an apparently autosomal dominant inheritance. 11 The underlying molecular defect is thought to affect connective tissue development, given than IA has been reported to be part of the phenotype in several human inherited connective tissue abnormalities.…”

“…In a series of 485 SAH patients, Norrgard et al 2 identified by means of a questionnaire survey 32 patients (6.6%) with a first-to third-degree relative who had a SAH. The number of relatives at risk and the method of confirming the diagnosis in these relatives were not reported.…”

“…1210 However, multiplicity varies considerably from family to family; in one retrospective study it was as high as 53%. 2 Even familial IAs seldom occur under the age of 20 years. 11 So far, only seven such patients have been reported, the youngest being 4 years old, 511 -15 which must be taken into account in decisions about elective screening.…”
