Intracorporeal circular-stapled Billroth I anastomosis in single-incision laparoscopic distal gastrectomy

Abstract: Our modified technique for intracorporeal Billroth I reconstruction in single-incision laparoscopic distal gastrectomy is safe and feasible. This technique could be an attractive surgical option for all laparoscopic reconstructive procedures using circular stapling devices.

“…In addition, it is more difficult to expect such advanced-assistance dependent procedure in SIDG, and one more small assistance grasper is likely to result in more collision because it makes narrow port space more crowded. Therefore, there have been limited number of original experience or modification of Billroth I anastomosis after SIDG, and reproducibility of gastroduodenostomy in pure SIDG still seems to be doubtful (20,23,24). We reported our novel technique for gastroduodenostomy, "unaided delta-shaped anastomosis", without any additional port or intracorporeal assistance for pure SIDG, and also hope this technique will contribute to reproducible establishment of gastroduodenostomy in SIDG after validation (17).…”

“…SILS is rarely applied for gastric cancer surgery. To the best of our knowledge, there have been only three reports describing this procedure for patients with EGC [5][6][7]. Furthermore, all these reports involve SIDG with one or two additional assistant ports.…”

“…Although NOTES is still being studied because of the limitations of current instruments, SILS has been frequently applied in various clinical settings, such as cholecystectomy, appendectomy, colectomy, and sleeve gastrectomy for obesity [3,4]. However, there have been only three reports about single-incision laparoscopic distal gastrectomy (SIDG) for gastric cancers [5][6][7]. Herein, we report a technical note of the first successful single-incision laparoscopic total gastrectomy (SITG) with D1?beta lymph node dissection (LND) for a patient with early proximal gastric cancer.…”

“…So far, there have been a few reports on RPS for gastrectomy for gastric cancer (1,2). However, it has remained unclear that RPS has the advantages compared with LAG, and this special technique has not been spread even more.…”
