Intracerebral haemorrhage volume, haematoma expansion and 3-month outcomes in patients on antiplatelets. A systematic review and meta-analysis

Abstract: Aims We assessed the association of prior antiplatelet therapy (APT) at onset of intracerebral haemorrhage (ICH) with haematoma characteristics and outcome. Methods We performed a systematic review and meta-analysis of studies comparing ICH outcomes of patients on APT (APT-ICH) with patients not taking APT (non–APT-ICH). Primary outcomes were haematoma volume (mean difference and 95% CI), haematoma expansion (HE), in-hospital 3-month mortality rates and good functional outcome (modified Rankin Scale score 0–2)… Show more

“…Although prior AP or warfarin use was associated with mortality and poor outcome in patients with SICH 34 , 35 , the impact of prior DOAC use on in-hospital mortality remains inconclusive 36 , 37 . A nationwide, cross-sectional Japanese analysis showed that prior use of DOAC (DOAC-ICH) was associated with significantly lower in-hospital mortality than prior use of warfarin (vitamin k antagonist (VKA)-ICH)) in SICH 38 .…”

“…According to some reports, 62,63 prior APT may increase the risk of HE in ICH patients. In a study on the effects of APT on ICH patients during the acute phase, Toyoda et al discovered that APT was a reliable indicator of acute HE (OR 7.67, 95% CI 1.62–36.4), emergent surgical evacuation of hematoma (OR 3.10, 95% CI 1.18–8.15), and acute death (OR 3.10, 95% CI 1.18–8.15) 63 .…”

“…32 ICH expansion is associated with death, 33 which probably explains why people taking an antiplatelet agent until the time of ICH have a higher risk of death and disability than people not taking antithrombotic agents. 34…”
