Intracerebral arachnoidal cyst containing choroid plexus

Abstract: A case of an intracerebral "primary" arachnoidal cyst of the left fronto-parietal region is reported. No communication with the ventricular system or with subarachnoidal space was found at the first surgical procedure. It contained genuine choroid plexus and clinically behaved as an expansive intracranial mass.

“…An entirely intracerebral arachnoid cyst with no communication with the ventricular system or with the subarachnoid space is extremely rare. To the best of our knowledge, only 4 cases have been reported in the English literature [3][4][5][6] (Table 1). We describe a case with a progressively enlarging intracerebral arachnoid cyst, which has been well documented by serial imaging over the preceding three years.…”

“…Others like CONTm~RAS [3], THOMPSON [11] and AD~LO~ [1) to mention just a few, have described identical cases. Besides these etiologies, other causes have been described by BuoY (1946 [2]), ROSICH-PIA (1977 [9]) and IGLESIAS (1981 [4]) in the form of ectopic choroid plexus with an active secretion of CSF. In our own material, we saw two such cases where histology supported the existence of choroid plexus in one case, without any relationship to the ventricles.…”

“…In the last 20 years, only 4 cases of epithelium-lined, noncommunicating cysts incorporating choroid plexus tissue or choroid-plexus-like tissue were reported [16, 17, 18, 19]. In 3 cases, the cysts were located supratentorially in a frontal, temporal or frontotemporal location not restricted to a subarachnoid cistern.…”

“…Especially, no clear statement concerning the composition of the cyst walls was made. A flattened cuboid epithelium was described similar to the pia mater or even the ependyma [17, 18]. Therefore, a clear explanation concerning the etiology of the papillary choroid-plexus-like structures found in the cyst walls was not given.…”
