Intracarotid Delivery of Drugs

Abstract: The major efforts to selectively deliver drugs to the brain in the last decade have relied on smart molecular techniques to penetrate the blood brain barrier while intraarterial drug delivery has drawn relatively little attention. In the last decade there have been rapid advances in endovascular techniques. Modern endovascular procedures can permit highly targeted drug delivery by intracarotid route. Intracarotid drug delivery can be the primary route of drug delivery or it could be used to facilitate the deli… Show more

“…4, 28-30 However, despite intensive efforts, IA drug delivery has yet to make a significant therapeutic impact in the treatment of brain cancers—a failure it shares with many other malignant brain tumor treatments. This is somewhat remarkable given that for cancers of other organs such as liver, breast, eye, and even extra-cranial tumors of the head and neck, IA chemotherapy is fairly effective.…”

“…In general, IA drugs are most effective in three specific situations: 1. when there is low regional blood flow, 2. when there is high regional drug extraction, and 3. when the drug is rapidly cleared from the systemic circulation. 30 In addition, more consistent tissue drug concentrations are achieved by pulsed or bolus dosing. 30, 35, 36 Therefore, our approach to IA delivery to the brain uses transient blood flow reduction with concurrent intracarotid bolus injection.…”

“…Although an attractive idea, IA drug delivery has not found widespread clinical application for brain tumor treatments [2, 3, 11]. A fundamental challenge to IA drug delivery is that drugs must be extracted efficiently as they transit rapidly through a very well perfused tissue.…”

“…The ultra-fast first-pass kinetics of IA drug injections are beyond the temporal resolution of conventional pharmacokinetic methods, such as micro-dialysis and tissue biopsy [11]. Therefore, we used a specialized version of diffuse reflectance spectroscopy (DRS) to track brain tissue concentrations of cationic liposomes that were labeled with a near infrared dye, DilC 18 (7) [12–14].…”
