Intracardiac extension of intravenous leiomyomatosis.

Abstract: A 42-year-old woman was found to have intravenous leiomyomatosis of the uterus with extension into the inferior vena cava and right atrium. Intravenous leiomyomatosis is a rare neoplastic disease characterized by invasion of venous channels by a benign smooth muscle tumor arising either from the wall of a vessel or from a uterine myoma. Intracardiac extension is often initially misdiagnosed as a right atrial myxoma and may cause death by mechanical obstruction. The diagnosis of intravenous leiomyomatosis shoul… Show more

“…However, enhanced CT showed intraatrial and intracaval mass filling defects [7][8][9][10][11], and pre-contrast and post-contrast MR imaging showed a mass with mild or strong enhancement in the right heart and inferior vena cava [6][7][8][12][13][14]. Thus, it was not difficult for us to differentiate intravenous leiomyomatosis from our case, which was a cystic structure mimicking inferior vena cava dissection.…”

“…There have been previously reported studies involving a uterine leiomyoma or leiomyosarcoma extending into or simulating an atrial myxoma [10,11,12, 15]. Nevertheless, we were only able to identify one reported case documenting the treatment for both primary uterine cancer and an atrial myxoma [9].…”
