We report three cases of para-articular chondroma and osteochondroma in the region of infrapatellar fat pad. All three lesions were resected and examined histologically. Two of them were primarily cartilaginous with a lobular pattern internally, and one uniformly osseous with peripheral cartilage. We conclude that these lesions are not the same. The former should be designated para-articular chondroma after Jaffe and the latter, osteochondroma.Résumé Nous rapportons trois cas de chondrome paraarticulaire et d'ostéochondrome situés dans le corps adipeux infrapatellaire. Ces trois lésions ont été résequèes et examinées histologiquement. Deux d'entre elles étai-ent au départ cartilagineuses et avaient une forme lobulaire interne, et l'autre était uniformement osseuse avec un cartilage périphérique. Nous en concluons que ces lés-ions ne sont pas les mêmes histopathologiquement. Les premières devraient être appelées, à la suite de Jaffe, chondrome para-articulaire et la troisième, ostéochond-rome, de façon à l'opposer aux dénominations équivo-ques utilisées dans les présentations précédentes.

La chondromatose synoviale est une métaplasie ; elle entraîne la prolifération cartilagineuse du tissu synovial qui détermine la formation de chondromes pouvant s'ossifier (ostéochondromes). À notre connaissance, l'atteinte de la gaine des tendons fibulaires n'a jamais été décrite dans la littérature. Les auteurs rapportent l'observation d'un footballeur âgé de 36 ans qui se plaignait d'une tuméfaction de la région malléolaire latérale de la cheville gauche, sans douleur, mais avec gêne au chaussage. Après excision et synovectomie, le diagnostic de la tumeur retenu était une chondromatose synoviale de la gaine des tendons péroniers.Touchant souvent les grosses articulations, la chondromatose est rarement extra-articulaire. L'atteinte des gaines tendineuses se situe essentiellement à la main et au pied. L'aspect clinique et radiologique peut prêter confusion avec un chondrosarcome de bas grade. C'est dire l'intérêt d'un diagnostic précis confirmé par une étude anatomo-pathologique. Le traitement repose sur une excision de tous les chondromes, associée à une synovectomie afin d'éviter la récidive. Synovial chondromatosis of fibular tendon sheathAbstract: Synovial osteochondromatosis (SOC) is characterized by synovial membrane metaplasia with production of small cartilaginous nodules that protrude on the inner surface of the synovial membrane, undergo pedunculation, and finally break off. Free cartilage fragments may undergo ossification. To our knowledge, fibular sheath localization has not been previously described. The authors report a case of a 36 year-old football player, who presented a one year history of swelling over the malleolar region of the left ankle, without pain or functional deficit. After excision and synovectomy, the diagnosis was a synovial chondromatosis of the fibular tendons sheath.The chondromatosis involves large articulations and the extra articular form is rare. The involvement of tendinous sheaths concerns usually the hand and foot. The clinical and radiological features may be mistaken for low grade chondrosarcoma. For an accurate diagnosis, the histological confirmation is important. The excision of loss bodies with synovectomy permits to avoid the recurrence.
