Purpose: to clinically and radiographically evaluate revascularization induced maturogenesis of non-vital immature teeth using Platelets rich plasma (PRP) compared to blood clot as scaffolds with two different intracanal medications [modified triple antibiotic paste (mTAP) and calcium hydroxide (Ca(OH) 2 ) mixed with chlorhexidine (CHX)]. Materials and Methods: 32 patients aged between 9 and 20 years requiring endodontic therapy of maxillary central incisors were selected for this study. Patients were randomly divided into four equal groups (n=8) according to type of scaffold and intracanal medication used in the treatment as; Group I: Blood clot scaffold/ mTAP intra canal medication ,Group II: PRP scaffold/mTAP intra canal medication, Group III: Blood clot scaffold/Ca(OH) 2 mixed with CHX intra canal medication, Group IV: PRP scaffold/ Ca(OH) 2 mixed with CHX intra canal medication. Patients were evaluated clinically for presence or absence of spontaneous pain, pain on palpation, swelling, sinus tract or crown discoloration and radiographically for any change in apical foramen diameter, root length and root canal diameter at a predetermined point at 15mm from the incisal edge. Radiographic digital examination was done immediately postoperative and at 3,6,9 and 12months of follow-up under the same circumstances. Results: Clinically, all patients were completely asymptomatic through out the study. Radiographically, concerning percentage of root length increase, PRP produced superior results with statistically significant difference compared to blood clot as a scaffold while mTAP showed significantly better results compared to Ca(OH) 2 mixed with CHX as intracanal medication. Concerning percentage of canal diameter decrease and apical foramen diameter decrease, using PRP scaffold with mTAP medication produced superior results with statistically significant difference when compared to blood clot scaffold with a mix of Ca(OH) 2 and CHX medication respectively. Conclusions: PRP is better than Blood clot as a scaffold and mTAP is better than Ca(OH) 2 mixed with CHX as an intracanal medication when they are used during revascularization of immature teeth.

Root canal treatment is incomplete without usage of intra canal medicaments. They help in the reduction of bacterial count and its by-products, making canals clean and decreasing postoperative pains. The aim of this study was to evaluate the effect of calcium hydroxide (Ca [OH]2) and triple antibiotic paste on matrix metalloproteinase-9 (MMP-9) and vasoactive intestinal peptide (VIP). Thirty single rooted teeth with periapical lesions which require root canal treatment were randomly divided into 2 groups; Group 1-calcium hydroxide and Group 2-triple antibiotic paste. After access opening and working length determination, cleaning and shaping of the root canals were done and intracanal dressing was given. In the second appointment, medicaments were removed, and sampling was performed by collecting the interstitial fluid of the apical tissues by paper points to assess the MMP-9 and VIP levels. The MMP-9 and VIP levels were measured by the enzyme-linked immunosorbent assay. The concentration of MMP -9 on Ca(OH)2 and TAP was 27.3pg/ml and 29.2pg/ml and VIP on Ca(OH)2 and TAP was 27.9pg/ml and 28.4pg/ml. There was no statistically significant difference between the intracanal medicament on the MMP-9 and VIP (p>0.05), proving that type of intracanal medicament does not influence MMP 9 and VIP levels.

Calcium hydroxide is often used as a root canal medicament. Together with vehicle aqueous, glycerine, polyethylene glycol, and chitosan can increase calcium ion diffusion, pH balance, and anti-bacterial power. To analyze the effect of different vehicles on alkaline pH changes, diffusion of calcium ions, roughness, and frequency of chemical elements in the root canal after applying calcium hydroxide. A total of 100 dental samples in 5 groups of vehicles were examined for pH using a pH meter, calcium ion diffusion using Atomic Absorption Spectrophotometric, and surface roughness of the root canals chemical compounds were analyzed by SEM-EDX. All the vehicles can maintain the balance of pH changes to alkaline in the root canal wall (p<0.05: 0.00). Maximum calcium ion diffusion occurs on day 30, especially in the vehicle Chitosan and Polyethylene glycol (p <0.05: 0.00). Vehicle chitosan and glycerine more strongly reduce root surface wall roughness after calcium hydroxide medicament (20% roughness score). Specifically, vehicle chitosan can increase calcium (21.71%) compared to control and other vehicles in the root canal wall (p<0.05: 0.02). The chitosan vehicles and other vehicles can maintain an alkaline pH balance, diffusion of calcium ions, and decrease in roughness and increase in the quantity of calcium in the root canal wall.
