Intra-articular triamcinolone hexacetonide in knee osteoarthritis: factors influencing the clinical response.

Abstract: Objective-To assess the efficacy of a single intra-articular injection of triamcinolone hexacetonide (THA) in knee osteoarthritis (OA) and examine factors which may relate to treatment efficacy. Methods-Eighty four patients with clinical and radiographic evidence ofknee OA were recruited and randomly allocated to receive either THA (20 mg in 1 ml) or placebo (0.9% normal saline, 1 ml).

“…The durability of benefit of intraarticular therapies is limited. In placebo-controlled clinical trials, the duration of benefit following corticosteroid injection is 1-8 weeks; for hyaluronate, the expected benefit is 12-52 weeks (8)(9)(10)(11)(12). Furthermore, the effectiveness of each of these treatment options is limited.…”

“…USG guidance was not used in any of the previous studies evaluating the effect of IA corticosteroids in knee osteoarthritis [4,5]. Moreover, the applied dose was smaller than the ACR recommended dose [12][13][14]. Application of the recommended dose under USG guidance may have ameliorated the analgesic effect in this study even the patient group had advanced osteoarthritis.…”

“…Gaffney et al [12] conducted a study to compare 20 mg IA triamcinolone hexacetonide with placebo in knee osteoarthritis and reported significant improvement in pain with triamcinolone at the end of the first week. However, there was no difference between the groups at the end of 6 weeks.…”

“…54 Nearly 80% of patients report improvement at 1 week. The clinical benefit is improved when effusion is present and aspiration of synovial fluid at the time of injection is successful (recommendation strength B).…”

“…The clinical benefit is improved when effusion is present and aspiration of synovial fluid at the time of injection is successful (recommendation strength B). 54 It is not clear that frequent injections will damage articular cartilage or the joint's structure. In common practice, however, intra-articular steroid injections are limited to 3 to 4 per year (recommendation strength C).…”
