Intra-articular fractures of the calcaneum. Part II: Open reduction and internal fixation by the extended lateral transcalcaneal approach

Abstract: The classification of intra-articular fractures of the calcaneum described in part I is related to an operative approach which allows accurate reduction and stable fixation of the fracture fragments. An extended lateral incision is used to avoid sural nerve damage and problems of soft-tissue healing. In type 3 fractures, access to the lateraljoint fragment requires an osteotomy of the lateral wall, but after this the lateral joint fragment can be rotated out of the subtalar joint to allow transcalcaneal reduct… Show more

“…Only the accurate reduction with re-establishment of statics and function of the foot allows normal biomechanics [4,29,31,32]. These can only be achieved by surgical treatment.…”

“…Various approaches from the medial or the lateral aspect have been developed [1,9,22]. In our experience, the exclusive L-shaped, extended lateral approach with en-bloc mobilization of a proximal skin and soft tissue flap offers good orientation, permits reduction of the depressed posterior articular fragment and allows stable fixation with plate and screws while preserving soft tissue vascularity [5,28].…”
