Intra-abdominal patterns of disease dissemination in colorectal cancer identified using radioimmunoguided surgery

Abstract: RIGS detected more tissue involved in disease process for all sites in both primary and recurrent disease except liver metastases. Areas with highest proportion of RIGS-positive tissue, the gastrohepatic ligament and celiac nodes, are rarely resected and are not pathologically examined. Positive RIGS localization of tumor antigen in these areas suggests more extensive dissemination of disease process.

“…A review of trials from 1986 to 1991 showed that the use of RIGS for recurrent colorectal cancer leads to detection of 79–100% of sites [ 3, 7, 8, 11, 15–22]. Additional information has been provided in 0–90% of patients, leading to alterations in the operative procedure in 16–60% of cases, either by extending the planned resection or by avoiding unnecessary surgery.…”

“…These trials have shown that RIGS technology provides information concerning the extent of disease and dissemination of tumor tissues (cells and by-product antigens) [7]. RIGS-identified tissues are helping to explain the cascade of events that lead to metastasis.…”

“…A number of studies have been done with the anti-TAG MoAb, most of them at Ohio State University and James Cancer and Research Institute, Columbus, Ohio. RIGS technology revealed patterns of tumour dissemination [19,20]. The surgeon was able to locate cancer and find micrometastases in LNs that were negative for tumour after routine H&E pathological examination.…”
