Intra-abdominal insertion of sewing needles: a rare method of child abuse

Abstract: The insertion of sewing needles into the abdominal cavity is an uncommon form of child abuse. We report a 2-and-a-half-year-old boy with 2 intra-abdominal sewing needles that were discovered during the evaluation of chronic abdominal pain and vomiting. This case report illustrates the wide range with which abusive injury can present in children. Pediatricians need to be alerted to this newly-recognized and the increasing form of child abuse so that they examine and diagnose their patients appropriately.

“…Child abuse with needles has been described and so has been the need for laparotomy in such cases. [ 9 ] However, the case presented here (case 3) is only the second reported in the literature of a noningested intraperitoneal/visceral FB extracted with laparoscopy. [ 10 ] A timely laparoscopy in these cases should be the standard practice to avoid potentially fatal complications.…”

“…Such cases have been reported from across the world with different lodging sites in the body, most frequently in the cranium, among other sites like abdomen, limbs, neck, scrotum, and buttocks etc. [27] However, these are the first 2 cases to our knowledge where a needle was found in the chest/lung as a result of suspected child abuse. Besides the site of the needle, the ages of the patients were also quite different from the other cases reported.…”

“…There are few documented cases of sewing needles causing trans-abdominal penetration, whether accidentally or deliberately from self-injury or abuse. 1,2 Usually, gastrointestinal perforation by foreign bodies occurs usually after ingestion and not from trans-abdominal penetration. 3,4 Trans-abdominal penetration with sharp foreign bodies usually present acutely even without acute abdominal pain; if there are abdominal symptoms or signs, the indication for exploration becomes mandatory.…”
