Intestinal phase of gastric secretion in patients with duodenal ulcer

Abstract: SUMMARY In 10 healthy subjects and 10 duodenal ulcer patients the intestinal phase of gastric acid secretion was studied by intraduodenal infusion of a 10 % liver extract meal (pH 7) at 400 ml/h for three hours. A gastroduodenal double lumen tube with two balloons was used to block the pylorus and to prevent duodenogastric reflux. Gastric acid response to a duodenal meal ofliver extract reached a peak at the end of the first hour of infusion of the extiact and was then followed by a relatively well-sustained p… Show more

“…It is of interest that intestinal phase stimulation of gastric secretion by liver extract meal instilled into the duodenum in animals (Konturek, 1977) and by amino acid mixture in man (Konturek et al, 1978) caused a marked augmentation of the maximal response to pentagastrin or histamine-probably due to interaction of specific intestinal phase hormone, entero-oxyntin, with these exogenous stimulants. Again patients with duodenal ulcer exhibited more marked and prolonged augmentation to the combination than healthy subjects.…”

“…However, in studies that demonstrated marked stimulation of acid secretion from a denervated gastric pouch by an intestinal meal, no increase in peripheral serum gastrin levels was detected by Debas et al [105] in dogs with an intact antrum, or by Way et al [106] in antrectomized dogs. Moreover, in 4 separate studies in humans, intraduodenal instillation of liver extract, peptone, or amino acids provoked substantial gastric acid secretion without an increase in serum gastrin concentration [113,114,117,118].…”
