Intestinal involvement in Behçet's disease

Abstract: A survey was prepared of 136 cases of laparotomy in patients with Behçet's disease and intestinal ulcers in Japan. The incidence of the disease is highest in males in the fourth and fifth decades. The ulcers are frequently found in the terminal ileum and the cecum. The ulcers are liable to perforate into the abdominal cavity, so that many patients require emergency operation. “…Among the 136 reported cases in the Japanese literature of patients with BD requiring laparotomy, abdominal pain was found in 92%, mass in 21%, and/or melena in 17% [17]. Almost all Japanese children with GI complications presented with abdominal pain, bloody stool, high fever, and anal lesions [18]. Of patients undergoing CT scanning, 82% had abdominal pain; 25% had weight loss; 18% had diarrhea, and 14% had melena or fever [19].…”

“…Since endoscopic investigations are usually performed in clinically symptomatic cases in BD, the exact prevalence of GI tract involvement in BD is unknown. There are several descriptions about the endoscopic characteristics of intestinal BD [1,4,5,6]. Most of the studies have reported the macro- and microscopic features of colonic lesions at colonoscopy [5,6], whereas lesions of the small intestine have only been defined in cases with perforated intestine.…”

“…Although gastrointestinal symptoms are common in Behçet's disease, the demonstration of gastrointestinal ulcers is rare [26]. In intestinal Behçet's disease, ulceration of the gastrointestinal tract can be found throughout the intestine, but the most frequent area of involvement is the ileocecal region, with extension into the ascending colon.…”
