BackgroundMany refugees have experienced the death of a loved one under traumatic circumstances. Accordingly, the prevalence of Prolonged Grief Disorder (PGD) among refugees is high. Culture-specific symptoms of PGD have been described previously, but beliefs about causes and cures of PGD among refugees remain unknown. We therefore aimed at identifying illness beliefs and treatment expectations regarding PGD among refugees.MethodWe focused on refugees from Arab countries (n = 14) and from Sub-Sahara Africa (n = 9) and applied qualitative and quantitative methods. In a semi-structured interview, participants first answered questions about assumed causes and potential cures for prototypical PGD symptoms according to ICD-11 that were presented in a vignette as representatives of their own culture. In the quantitative part, they completed the Cause Subscale of the Illness Perception Questionnaire (IPQ-R) that included additional culture-specific items. Interviews were analyzed with Qualitative Content Analysis.ResultsIn both groups of refugees, PGD symptoms were predominantly attributed to a close relationship to the deceased, lack of social support, personal vulnerabilities, and circumstances of the death. Participants also named a number of flight-related causes (e.g., inability to perform or participate in rituals, feeling isolated in the host country). None of the participants attributed PGD symptoms to supernatural causes. Descriptive analyses of responses on the IPQ-R indicated that participants predominantly attributed PGD symptoms to psychological causes. Participants believed that PGD can be cured and predominantly mentioned social and religious support. Psychological help was only mentioned by a minority of participants. In both groups, participants emphasized that a therapist must be familiar with the patient's culture and rituals. Participants also mentioned stigma associated with seeking psychological help.ConclusionResults suggest specific beliefs of refugees regarding causes and cures of PGD as well as similarities with Western conceptualizations. A culture-sensitive approach to the treatment of PGD in refugees that can include knowledge of culture-specific rituals and incorporating religious beliefs as well as decreasing stigma and increasing mental health literacy seem important. The study is limited by its focus on only two groups of refugees and its small sample size.

Second opinion programmes in Germany: a mixed-methods study protocol

IntroductionSecond opinion programmes aim to support the patients’ decision-making process and to avoid treatments that are unnecessary from a medical perspective. The German second opinion directive, introduced in December 2018, constitutes a new legal framework in statutory health insurance for seeking second opinions for elective procedures and so far includes tonsillectomy, tonsillotomy, hysterectomy and shoulder arthroscopy. The directive mandates physicians who recommend one of the above-mentioned surgeries to inform their patients of their legal right to visit a certified second opinion provider. Since second opinion programmes are a fairly recent phenomenon in Germany, no comprehensive data are yet available on the degree of implementation, users, potential barriers and their effectiveness. We aim to examine the characteristics and the use of second opinion programmes as well as the needs and wishes from the perspective of (potential) users in Germany, with focus on the decision-making process, the patient–physician relationship and the motivation to seek a second opinion, as well as the role of health literacy.Methods and analysisSix substudies will include the following stakeholders: (1 and 2) patients with one of the four surgery-indications covered by the directive, (3) patients who electively sought an online-based second opinion, (4) patients with oncological diseases, (5) the general population and (6) medical specialists. A mixed-methods approach will be used, including questionnaires, interviews and focus groups. The data will be evaluated using quantitative descriptive analysis and qualitative content analysis. The integration of the results will take place in the form of a triangulation protocol.Ethics and disseminationThe study protocol was approved by the Ethics Committee of the Brandenburg Medical School. The findings will be published in peer-reviewed journals and presented at scientific conferences.

Turning points describe a fundamental change of direction in a life course trajectory. However, they are challenging to study because of their temporal extension, the complexity of processes at stake during those critical life sequences, and the fact that individuals’ interpretation as well as more objective changes in social status and position play important roles in turning points. Panel-based mixed methods designs are well suited to address those challenges. In-depth interview data enable researchers to understand individuals’ interpretations and offer detailed understanding of the processes at stake on multiple levels and domains. The panel survey data allow a glimpse into the respondents’ past, which can serve as a detailed resource for further case-based information. At the same time, the continuing survey data collection in prospective waves yields important data on respondents’ futures that can be analysed against the background of the told history. In this paper, we draw on data from a mixed methods study on the labour market trajectories of 23 descendants of immigrants in Germany, based on the Socio-Economic Panel survey (SOEP). The aim of the paper is to show the potential of combining retrospective interviews and panel data to account for the objective as well as subjective dimensions of turning points. Combining qualitative data and panel data does not aim at reaching the ‘right’ understanding of each case but at obtaining a multifaceted picture of respondents’ lives, which can help to avoid misinterpretation and under-theorisation. Key messages <ul><li>Turning points are challenging to study with common designs. Panel-based mixed methods designs offer a solution.</li> <li>They enable studying sequences in detail and also offer insights into individuals’ experiences.</li> <li>They extend data collection into the future; a crucial prospect for analysing processes across respondents’ lives.</li></ul>
