Objective: To evaluate the incidence of a software program package at Brazilian public eye care institutions through an online survey. The package is made up of the institutions' websites, electronic medical records, regulatory assistance control system and the preoperative scheduling system. The aim is to analyze if this RESUMOObjetivo: Avaliar, através de busca na internet, quais os serviços públicos com atendimento oftalmológico no Brasil que possuem um pacote digital composto de página da instituição na rede digital, prontuário eletrônico, sistema de regulação de atendimento e fila de espera cirúrgica digital. Enfatizar a presença destes elementos de pesquisa com a condição de funcionamento de fila de espera cirúrgica digital com possibilidade de consulta publica pelo paciente usuário. Métodos: Estudo transversal através de busca na internet de serviços de oftalmologia com este pacote digital completo ou parcial em caráter nacional. Os dados foram pesquisados na internet no período de junho à julho de 2015. As instituições foram divididas em grupos de pesquisa, assim designados: hospitais multidisciplinares universitários, hospitais multidisciplinares assistenciais, hospitais uni-disciplinares assistenciais (somente oftalmologia) e banco de olhos (como entidade de atendimento público). Duas variáveis foram avaliadas: número total e parcial de serviços públicos de oftalmologia que possuem este sistema digital e como foi desenvolvido este pacote digital por cada instituição. Os dados foram avaliados através do teste Qui quadrado e prova exata de Fisher. O nível de significância do estudo foi 0,05. Resultados: Foram encontradas 48 instituições públicas oftalmológicas detentoras deste pacote digital, sendo: 24 universitárias, 15 assistenciais, 3 hospitalares uni-disciplinares (somente oftalmologia), 6 bancos de olhos. Somente 10,4 % destas instituições apresentaram uma fila de espera cirúrgica digital. Exclusivamente os bancos de olhos obtiveram dados significativos quanto a presença de fila cirúrgica digital, bem como a origem de seu desenvolvimento. Conclusão: Apenas algumas instituições apresentaram um pacote digital completo, porém a fila de espera cirúrgica digital é o componente mais raro destas instituições. Razões desta constatação são explanadas pelo estudo. Embora com pequena representação, em todo território nacional somente dois bancos de olhos apresentaram fila cirúrgica digital com possibilidade de consulta pública.Descritores: Administração hospitalar/normas; sistemas computadorizados de registros; registros eletrônicos de saúde; saúde pública; instituições de saúde; Brasil

BackgroundDelivery arrangements include changes in who receives care and when, who provides care, the working conditions of those who provide care, coordination of care amongst different providers, where care is provided, the use of information and communication technology to deliver care, and quality and safety systems. How services are delivered can have impacts on the effectiveness, efficiency and equity of health systems. This broad overview of the findings of systematic reviews can help policymakers and other stakeholders identify strategies for addressing problems and improve the delivery of services.ObjectivesTo provide an overview of the available evidence from up-to-date systematic reviews about the effects of delivery arrangements for health systems in low-income countries. Secondary objectives include identifying needs and priorities for future evaluations and systematic reviews on delivery arrangements and informing refinements of the framework for delivery arrangements outlined in the review.MethodsWe searched Health Systems Evidence in November 2010 and PDQ-Evidence up to 17 December 2016 for systematic reviews. We did not apply any date, language or publication status limitations in the searches. We included well-conducted systematic reviews of studies that assessed the effects of delivery arrangements on patient outcomes (health and health behaviours), the quality or utilisation of healthcare services, resource use, healthcare provider outcomes (such as sick leave), or social outcomes (such as poverty or employment) and that were published after April 2005. We excluded reviews with limitations important enough to compromise the reliability of the findings. Two overview authors independently screened reviews, extracted data, and assessed the certainty of evidence using GRADE. We prepared SUPPORT Summaries for eligible reviews, including key messages, 'Summary of findings' tables (using GRADE to assess the certainty of the evidence), and assessments of the relevance of findings to low-income countries.Main resultsWe identified 7272 systematic reviews and included 51 of them in this overview. We judged 6 of the 51 reviews to have important methodological limitations and the other 45 to have only minor limitations. We grouped delivery arrangements into eight categories. Some reviews provided more than one comparison and were in more than one category. Across these categories, the following intervention were effective; that is, they have desirable effects on at least one outcome with moderate- or high-certainty evidence and no moderate- or high-certainty evidence of undesirable effects.Who receives care and when: queuing strategies and antenatal care to groups of mothers.Who provides care: lay health workers for caring for people with hypertension, lay health workers to deliver care for mothers and children or infectious diseases, lay health workers to deliver community-based neonatal care packages, midlevel health professionals for abortion care, social support to pregnant women at risk, midwife-led c...

The effect of hospital spending on waiting times

Long waiting times have been a persistent policy issue in the United Kingdom that the COVID‐19 pandemic has exacerbated. This study analyses the causal effect of hospital spending on waiting times in England using a first‐differences panel approach and an instrumental variable strategy to deal with residual concerns for endogeneity. We use data from 2014 to 2019 on waiting times from general practitioner referral to treatment (RTT) measured at the level of local purchasers (known as Clinical Commissioning Groups). We find that increases in hospital spending by local purchasers of 1% reduce median RTT waiting time for patients whose pathway ends with a hospital admission (admitted pathway) by 0.6 days but the effect is not statistically significant at 5% level (only at the 10% level). We also find that higher hospital spending does not affect the RTT waiting time for patients whose pathway ends with a specialist consultation (non‐admitted pathway). Nor does higher spending have a statistically significant effect on the volume of elective activity for either pathway. Our findings suggest that higher spending is no guarantee of higher volumes and lower waiting times, and that additional mechanisms need to be put in place to ensure that increased spending benefits elective patients.
