A 38-year-old army officer started therapy in 2020 with a four-year history of auditory hallucinations and delusions of reference, persecution and grandeur, symptoms that were resistant to traditional antipsychotic medications. He follows an integrative psychotherapy program that aims to reduce his anxiety, continues his antipsychotic medications, and has Infra-Low Frequency Neurofeedback. After his initial assessment he had a 40 min session of Infra-Low Frequency Neurofeedback before any other kind of intervention. Before and immediately after the session he completed the SCL-90 scale and the Visual Analog Scale covering 20 aspects of his psychological and physical state as well as his schizophrenic symptoms. This first Neurofeedback session had dramatic effects on his psychotic symptoms, levels of anxiety and psychosomatic condition, before his first psychotherapy session and/or any changes in his antipsychotic medication. The above results have great importance due to the severity and chronicity of schizophrenia. Informed consent was obtained from the participant for the publication of this case report (including all data and images).

Psychotherapy Process Research in Schizophrenia Paranoid Type: The Investigation of Delusion Formation through the Evaluation of In-Session Events

The study aimed to investigate delusion formation in paranoid type schizophrenia through the evaluation of the relation between specific in-session events and the session outcome. Six transcripts of integrative psychotherapy sessions -three with good and three with bad outcome- of an individual suffering from paranoid type schizophrenic symptoms were evaluated by five raters in order: a) to locate the in-session events related to delusion formation and b) to explore the relation of these events to a number of mechanisms postulated to be involved in delusion formation, utilizing the newly developed Scale for the In-session Investigation of Delusion Formation. Although, no significant differences were found in the total number of the in-session events counted in the sessions with good and bad outcome, the evaluations obtained by the raters were found to be affected by the patient’s general decrease in psychopathological symptoms due to a significant parallel reduction of anxiety and delusions in paranoid type schizophrenia.

An integrative psychotherapy model is described which proposes that the basic "schools" of psychotherapy, that is the psychodynamic, behavioral, humanistic/existential, systemic and biological approaches can be synthesized into a new improved paradigm. This total holistic approach is not in conflict with any of the contributing models which are perceived as complementary to each other. Moreover, there is ample scientific evidence that all traditional psychotherapies share the same common therapeutic factors. Hellenic integrative psychotherapy comprises a new idealistic approach which has different improved properties in the same way that Water differs from its constituents Oxygen and Hydrogen. Elements of ancient Greek philosophy, mythology, history, literature, ethics, values, reasoning, mentality and way of life are considered to be an important constituent of Hellenic Integrative Psychotherapy. Hippocratic diet, Bodily exercises that enhance physical wellness and brain function, Therapeutic thermal and cold baths, Thalassotherapy, Exposure to sunlight, Music therapy, Theater therapy, Art therapy, Therapeutic architecture and the exposure to a Therapeutically beautiful physical environment constitute important integral elements of the presented total holistic approach. Last but not least, Neurofeedback (training of the EEG) and Event Related Potentials (ERPs) will be employed.
