Background:Falling is an age-related problem that increases with age. Compared with younger people, elderly people possess increased risk factors for falls, and falling among the elderly is associated with increased mortality. Risk factors for falls have been reported in elderly outpatients; however, whether sagittal spinal posture affect the risk of falls in community residents remains unclear. Therefore, we aimed to investigate the influence of sagittal spinal posture on the risk of falls in elderly community-dwelling people using spino-plevic sagittal parameters in a retrospective longitudinal study. Methods:A total of 463 volunteers (96 men and 367 women; mean age, 72.8 years) who underwent a routine physical checkup were evaluated. Baseline whole spine and lower limb radiography, physical tests, bone mineral density (BMD), number of medications and comorbidities, patient-reported outcomes (PROs), and a history of falls in the previous four years period were examined.Results: Univariate analysis revealed older age, lower height and weight, higher prevalence of vertebral fractures, higher number of medications, poor physical test scores including one-leg standing test and prone trunk extension, poor PROs, a higher sagittal vertical axis, and higher pelvic tilt (PT) as factors significantly associated with the risk of falls, and multivariate analysis revealed a higher sagittal vertical axis [odds 2 ratio (OR), 1.08; 95% confidence interval (CI), 1.002-1.013; P = 0.02] and locomotive syndrome assessed using the 25-Question Geriatric Locomotive Function Scale score (OR, 1.028; 95% CI, 1.004-1.053; P = 0.03) to be associated with the risk of falls, independent of other factors in the univariate analysis. Conclusions:The sagittal vertical axis was an independent risk factor for falls, and the prevalence of vertebral fractures and prone truck extension correlated with the sagittal vertical axis. Prospective and intervention studies are needed to prevent future falls in elderly community volunteers with a higher sagittal vertical axis.

BackgroundChronic obstructive pulmonary disease (COPD) is a major cause of disability. We aimed to analyse the impact of reduced pulmonary function on non-respiratory impairments and mobility activity limitations in an elderly population with COPD and to elucidate which specific limitations on mobility are related to reduced pulmonary functionMethodsCross-sectional study of 110 patients with COPD, recruited from public and university hospital. The effect of impaired pulmonary function on the risk of non-respiratory impairments and mobility limitations was analysed using validated measures, including: the 6-Minute Walk Test (6MWT), skeletal muscle strength, the Short Physical Performance Battery (SPPB), and self-reported mobility questionnaire. Multivariate analysis was used to control for confounders such as age, sex, height, education, and cigarette smoking.ResultsGreater impairment of pulmonary function was associated with less distance walked during the 6MWT, poorer SPPB scores, and greater risk of self-reported mobility limitations (p<0.05). Lower forced expiratory volume in 1 s was also associated with a greater risk of limitations in carrying items under 10 pounds (4.54 kg), walking alone up and down a flight of stairs, and walking two or three neighbourhood blocks. There was no clear statistical relationship between pulmonary function impairment and skeletal muscle strength.ConclusionsImpaired pulmonary function was associated with the 6MWT score and limitations on performance-based and self-reported mobility activities, but not with skeletal muscle strength among elderly COPD patients.

Background Individuals with advanced osteoarthritis (OA) of the knee experience significant impairments in balance and in essential physical functions such as walking and rising from a chair. There is limited evidence on valid outcome measures to capture these impairments. Questions/Purposes We sought to examine the construct validity of three physical performance measures in patients with advanced knee OA: a gait speed (GS) test, the Short Physical Performance Battery (SPPB), and the Timed Up and Go (TUG) test. Methods We designed a cross-sectional clinical measurement study in which patients with advanced knee OA completed two self-reported measures: the Knee Injury and Osteoarthritis Outcome Score—Physical Function Shortform (KOOS-PS) and a four-part numeric pain rating scale (Q-NPRS). They were also administered the GS test, TUG test, and SPPB. Convergent and divergent construct validity were assessed by examining relationships between the GS test, the SPPB, the TUG test, the KOOS-PS, and the Q-NPRS and calculating Pearson correlation coefficients (r). The scores for the GS, TUG test, and SPPB were compared with established normative values for age-matched healthy controls. Results Forty-four subjects (mean age, 66.9 ± 8.1 years) participated in the study. The GS test showed low concordance with the SPPB component tests and the TUG test. The relationships between the physical performance measures and the self-reported measures were low. The scores for the GS test, TUG test, and SPPB in our sample were significantly worse when compared with age-matched normative values, indicating impairments in physical performance. Conclusion These results advance the understanding of the validity of the GS test, TUG test, and SPPB in demonstrating the impairments in physical performance that patients with advanced knee OA experience in walking, balancing, and rising from a chair. Future research should examine the reproducibility and responsiveness of the GS test, TUG test, and SPPB in patients with advanced knee OA, in order to facilitate the integration of these measures into clinical practice.
