International Society of Nephrology Global Kidney Health Atlas: structures, organization, and services for the management of kidney failure in Africa

Abstract: Despite positive economic forecasts, stable democracies, and reduced regional conflicts since the turn of the century, Africa continues to be afflicted by poverty, poor infrastructure, and a massive burden of communicable diseases such as HIV, malaria, tuberculosis, and diarrheal illnesses. With the rising prevalence of chronic kidney disease and kidney failure worldwide, these factors continue to hinder the ability to provide kidney care for millions of people on the continent. “…The high cost of PD must often be borne out‐of‐pocket by patients, and so is one of the most important challenges to increased use in LLMICs. In several African, 13 Latin American 31 and South Asian countries, 32 the annual cost of PD is much higher than HD and in some countries like India 32 and Egypt, 33 the cost of PD is almost double and nearly five times, respectively, of HD costs. High cost and excessive OOPPS for kidney care in Africans have been shown to be associated with high rates of mortality in those needing KRT 34 .…”

“…However, data from China showed a massive increase in PD utilization that rose from 8.6% in 2000 to 15.5% in 2012 along with increase in PD centres from <10 to more than 700 in the same period, a policy initiated to improve KRT access at affordable rates 12 . Only a few African countries have PD as an option for KRT despite having some of the lowest availability of KRT across all world regions 8,13 . This review describes various barriers to PD use in LLMICs and suggest ways to increase its utilization.…”
