“…In contrast, anticonvulsants were present at any level in 21% of BD suicide deaths, most commonly being carbamazepine (14%), with the other anticonvulsants below the privacy threshold. The high number of deaths in which carbamazepine was present was an unexpected finding, especially given the declining use in clinical practice for BD, which is currently estimated at 3% of BD patients (Mauer et al 2014 ). There is a modest literature suggesting higher rates of suicide attempts during treatment with carbamazepine monotherapy compared to lithium monotherapy (Cipriani et al 2013 ; Goodwin et al 2003 ), but other factors such as relative toxicity in overdose (Spiller et al 1990 ) and profile of patients who are prescribed carbamazepine (Leon et al 2012 ) may also be at play.…”

“…However, negligence of BAEs is especially impressive in the case of the use of APs in BDs patients. Although an increasing number of people affected by these disorders are being treated with these kinds of drugs (Pillarella et al 2012 ; Mauer et al 2014 ), the lessons learned from schizophrenia about antipsychotic-related BAEs do not seem to have influenced this clinical field. As shown in this review, there are no studies exploring the subjective and behavioral impact of these drugs and their possible therapeutic and functional consequences on patients with BDs.…”

“…The use of antidepressant medications is a controversial issue and perhaps more so in BD II because depressive features are so much more prominent. However, expert opinion recommends caution in their use (Mauer et al, 2014; Pacchiarotti et al, 2013a). The risks of rapid cycling, mixed states and switching into mania, are potential consequences, and efficacy is sometimes disappointing.…”
