Background In vivo and ex vivo simulation training workshops can contribute to surgical skill acquisition but require validation before becoming incorporated within curricula. Ideally, that validation should include the following: face, content, construct, concurrent, and predictive validity. Methods During two in vivo porcine surgical training workshops, 27 participants completed questionnaires relating to face and content validity of porcine in vivo flap elevation. Six participants’ performances raising a pedicled myocutaneous latissimus dorsi (LD) flap in the pig (2 experts and 4 trainees) were sequentially and objectively assessed for construct validity with hand motion analysis (HMA), a performance checklist, a blinded randomized procedure-specific rating scale of standardized video recordings, and flap viability by fluorescence imaging. Results Face and content validity were demonstrated straightforwardly. Construct validity was demonstrated for average procedure time by HMA between trainees and experts (p = 0.036). Skill acquisition was demonstrated by trainees’ HMA average number of hand movements (p = 0.046) and fluorescence flap viability (p = 0.034). Conclusion Face and content validity for in vivo porcine flap elevation simulation training were established. Construct validity was established for an in vivo porcine latissimus dorsi flap elevation simulation specifically. Predictive validity will prove more challenging to establish. Zusammenfassung Hintergrund Die Mikrochirurgie stellt eine breite und sehr anspruchsvolle chirurgische Teildisziplin dar, deren Ausbildung sich in Richtung Simulationstraining verschoben hat. Derzeit werden eine Reihe von Schulungsprogrammen für angehende Mikrochirurgen angeboten, die unterschiedliche Philosophien verfolgen. In dieser Studie stellen wir ein modulares Kurskonzept vor und evaluieren das Teilnehmerfeedback. Methoden Durch unseren modularen Lehrplan mit 4 Schritten können die Teilnehmer das Programm auf ihre individuellen Bedürfnisse zuschneiden und bestimmte Aspekte ihrer Ausbildung vertiefen. Das Hauptkonzept des Programms ist, dass Übungen angefangen von grundlegenden Techniken an biologischen avitalen Modellen bis hin zu spezifischen chirurgischen Techniken an menschlichen anatomischen Präparaten angeboten werden. In dem Teilnehmerfragebogen wurden die Parameter „Kursdesign“, „Erwartungen“ und „globale Zufriedenheit“ anhand einer Likert-Skala mit 5 Items (100 % – 75 % – 50 % – 25 % – 0 %) erhoben. Ergebnisse Von 2015 bis 2017 haben 274 Teilnehmer einzelne Kurse absolviert und bewertet. Die durchschnittlichen Werte lagen bei allen 3 beurteilten Parametern bei über 90 %. Diskussion Zusammenfassend haben wir ein innovatives Trainingsprogramm mit konstant hoher Zufriedenheitsrate und steigender Teilnehmerzahl entwickelt, das zur Verbreitung mikrochirurgischer Fähigkeiten in Mitteleuropa beiträgt.

“…The minimum requirement for a basic 5-days microsurgical training course, based on International Microsurgery Simulation Society guidelines, is to have 1 microscope per trainee and instruments—including 2 forceps (1 curved), curved needle holder, microsurgical scissors, vessel dilator, 9/0 and 10/0 sutures, and at least 2 microvascular clamps. 5 This equipment is proved to be costly, especially in developing countries, and during the early stages in skills acquisition, it can sustain damage from the interaction of untrained hands of inexperienced microsurgeons. 6 Permanent instrument damage may arise from an excessive application of pressure during tissue manipulation or from an inappropriate use that overpowers the instrument’s capabilities, resulting in the premature disposal of the unit.…”

Summary: Microsurgical training involves practice in ex vivo models during the early learning curve, and poor instrument handling by the inexperienced microsurgeons can cause damage to microsurgical instrumentation or clamps, which is particularly costly. To address this, we demonstrate the development, design, manufacturing, and application of 3 different types of 3-dimensional printed microvascular clamps in an ex vivo simulation training model. This report provides evidence of a low-cost and easily accessible device that facilitates the process of microsurgical training. The clamps were found to provide advantages akin to normal stainless-steel microvascular clamps in training settings.
