Internal Derangement of the Knee in Children and Adolescents

Abstract: Internal Derangement of the Knee in Children and Adolescents By H. A. T. FAIRBANK, M.S.THE purpose of this short paper is to call attention to the chief differences to be found when a series of cases of derangement of the knee-joint in young patients is compared with those of adult age. It is generally agreed that derangement calling for operation is rare in the young child, and that the frequency increases during adolescence and early adult life. “…The main diagnostic criteria of patellar chondropathy (group C) were pain when the knee functions under load in flexion, pain during prolonged sitting with the knee flexed and pain when the patella was compressed and rubbed against the femoral condyles (22,26,50). People with patellar subluxations and dislocations were excluded from the material on the basis of clinical history and the apprehension test (11). The diagnosis of iliotibial tract friction pain (group D) was based on a history of pain on the lateral femoral condyle after running a distance of several kilometers and on pain at palpation after running (46,48).…”

“…Lower limb alignment was normal, femoral torsion and tibial torsion were normal, passive and active range of motion were -5 to 140°, during knee extension the patella subluxated laterally (J-sign) [29] and the apphrension sign or Smillie test was positive [30].…”
