We carry out three-dimensional high-resolution numerical simulations of a bileaflet mechanical heart valve under physiologic pulsatile flow conditions implanted at different orientations in an anatomic aorta obtained from magnetic resonance imaging (MRI) of a volunteer. We use the extensively validated for heart valve flow curvilinear-immersed boundary (CURVIB) fluid-structure interaction (FSI) solver in which the empty aorta is discretized with a curvilinear, aorta-conforming grid while the valve is handled as an immersed boundary. The motion of the valve leaflets are calculated through a strongly coupled FSI algorithm implemented in conjunction with the Aitken convergence acceleration technique. We perform simulations for three valve orientations, which differ from each other by 45 deg and compare the results in terms of leaflet motion and flow field. We show that the valve implanted symmetrically relative to the symmetry plane of the ascending aorta curvature exhibits the smallest overall asymmetry in the motion of its two leaflets and lowest rebound during closure. Consequently, we hypothesize that this orientation is beneficial to reduce the chance of intermittent regurgitation. Furthermore, we find that the valve orientation does not significantly affect the shear stress distribution in the aortic lumen, which is in agreement with previous studies.

DZIELIMY SIĘ DOŚWIADCZENIEM WstępBadanie echokardiograficzne protez zastawkowych serca jest podstawową metodą oceny ich stanu. Pozwala ocenić funkcję protezy i wykryć zaburzenia związane z takimi patologiami, jak skrzepliny, wegetacje, tworzenie się łuszczki (łac. pannus), czy pojawianiem się patologicznych fal zwrotnych. Poza schorzeniami prowadzącymi do dysfunkcji zastawki należy pamiętać o możliwości ujawnienia się fabrycznych wad protez. Opis przypadkuMężczyzna, 50 lat, został skierowany do Pracowni Echokardiografii Kliniki Kardiologii Zachowawczej i Nadciśnienia Tętniczego CSK MSWiA w Warszawie z powodu stwierdzonej w kontrolnym badaniu echokardiograficznym przezklatkowym (ang. transthoracic echocardiography -TTE) dysfunkcji wszczepionej około pół roku wcześniej sztucznej zastawki aortalnej Medtronic Advantage 23 mm ze wstawką nadaortalną Vascutec 32 mm. Pacjenta zakwalifikowano do zabiegu z powodu istotnej hemodynamicznie złożonej wady aortalnej z poszerzeniem aorty wstępującej do 5,2 cm. Wymiar koń-coworozkurczowy lewej komory wynosił wówczas 79 mm, a frakcja wyrzucania lewej komory była obniżona do 40%. Przebieg okołooperacyjny był niepowikłany. W kolejnych badaniach echokardiograficznych (TTE) wykonanych w 3. i 5. miesiącu po zabiegu stwierdzono normalizację wymiaru lewej komory i poprawę kurczliwości jej ścian. Utrzymywały się nieco podwyższone gradienty ciśnień przez ujście aortalne odpowiednio 43/25 mm Hg i 50/28 mm Hg, co mogło wynikać z relatywnie wąskiego pierścienia protezy oraz niewielki centralny strumień niedomykalności interpretowany jako nieszczelność konstrukcyjna. W kolejnym, wykonanym w 6. miesiącu po zabiegu, badaniu echokardiograficznym stwierdzono pojawianie się nietypowej fali zwrotnej o umiarkowanym nasileniu. Influence of Valve Size, Orientation and Downstream Geometry of an Aortic BMHV on Leaflet Motion and Clinically Used Valve Performance Parameters

The aim of this study was to reconcile some of our own previous work and the work of others to generate a physiologically realistic numerical simulation environment that allows to virtually assess the performance of BMHVs. The model incorporates: (i) a left ventricular deformable model to generate a physiological inflow to the aortic valve; (ii) a patient-specific aortic geometry (root, arch and descending aorta); (iii) physiological pressure and flow boundary conditions. We particularly studied the influence of downstream geometry, valve size and orientation on leaflet kinematics and functional indices used in clinical routine. Compared to the straight tube geometry, the patient-specific aorta leads to a significant asynchronous movement of the valve, especially during the closing of the valve. The anterior leaflet starts to close first, impacts the casing at the closed position and remains in this position. At the same time, the posterior leaflet impacts the pivoting mechanisms at the fully open position. At the end of systole, this leaflet subsequently accelerates to the closed position, impacting the casing with an angular velocity of approximately -477 rad/s. The valve size greatly influences the transvalvular pressure gradient (TPG), but does not change the overall leaflet kinematics. This is in contrast to changes in valve orientation, where changing valve orientation induces large differences in leaflet kinematics, but the TPG remains approximately the same.

Abstract: In vitro intermittent regurgitation can be induced in the size 23 aortic ADVANTAGE valve under a limited range of conditions. To avoid possible misinterpretations, the phenomenon must be known in detail by all physicians dealing with patients with an ADVANTAGE valve.
