Intercostal Muscle Abscesses in Infective Endocarditis Associated With Migratory Deposition of Calcium Pyrophosphate

Abstract: Infective endocarditis (IE) is caused by vegetations, consisting of platelets, fibrin, inflammatory cells, and microcolonies of bacteria, fungi, rickettsia, chlamydia, and viruses, that form in the heart valves, endocardium, and large vessel intima. Staphylococcus aureus endocarditis is highly tissue destructive, usually follows an acute course, and tends to become severe due to valve destruction, surrounding abscesses, and distant seeding. The main complications of IE due to S. aureus are heart failure due to… Show more

“…The co-occurrence of MRSA infection and migratory pseudogout in this patient underscores the complexities in diagnosing and treating post-surgical complications in the elderly [4]. Migratory pseudogout, particularly outside joint manifestations, has been sparsely reported [5,6]. In our case, an older patient experienced migratory pseudogout amidst repeated MRSA infections at orthopedic surgical sites.…”

“…Causative organism was Pasteurella multocida [7]. Another case of IE with intercoastal muscle abscess caused by Staphylococcus aureus was reported by Nakayama et al [8].…”

“…Some publications report the formation of superficial muscle abscesses due to Staphylococcus aureus bacteremia. In muscles near joints, there are also reports that deposition of calcium pyrophosphate crystals, as observed in pseudogout, may result in pseudo abscess formation and increased susceptibility to infection [ 10 ]. It should also be recognized that S. aureus is the primary causative organism of infective endocarditis and that a wide range of clinical manifestations and musculoskeletal complaints of infective endocarditis are relatively common, with reports of initial manifestation of musculoskeletal symptoms that may present as pseudogout-like symptoms [ 11 ].…”
