Personality Traits in Migraine and Tension-Type Headaches: A Five-Factor Model Study

Although abnormal personality traits have been frequently reported in patients with primary headaches, the overlaps between these domains need a clear standard definition, for example, by a five-factor personality model. Moreover, personality abnormalities in patients with episodic tension-type headache (ETH) and migraine with aura (MA) are less well studied. Therefore, we administered Zuckerman-Kuhlman’s Personality Questionnaire to 72 patients suffering from chronic tension-type headaches (CTH), 33 with ETH, 15 with MA and 57 with migraine without aura (MO), as well as 58 healthy subjects. Depressive trends were measured with Plutchik-van Praag’s Depression Inventory. Compared to healthy controls, the CTH, ETH and MO groups showed significantly greater neuroticism-anxiety and depression. In addition, the MO group scored significantly higher on aggression-hostility than healthy controls. No abnormal personality traits were found in patients with MA. This study, from the perspective of a five-factor model, confirms most previous reports about personality in patients with primary headaches and establishes elevated aggression-hostility only in MO sufferers.

Depression influences personality measures like Eysenck's Big Three, Costa and McCrae's Big Five or Cloninger's Seven Factor models, and might also affect Zuckerman-Kuhlman's Personality Questionnaire (ZKPQ), an alternative five-factor model. The authors therefore tested ZKPQ in 85 patients suffering from major depression and in 82 healthy subjects in order to clarify this effect. Depressive mood was measured with Plutchik – van Praag's Depression Inventory (PVP). Patients scored significantly higher on PVP, Neuroticism-Anxiety and Aggression-Hostility, but lower on Activity and Sociablity than did healthy volunteers. In the general sample (N = 167), Neuroticism-Anxiety and Aggression-Hostility scores were positively correlated, while the Sociability score was negatively correlated with the PVP score. These results indicate that when the clinical significance relating to personality traits in patients is interpreted, depressive mood must also be considered.

Objective When English-speaking people listen to the Deutsch "high-low" word illusion, they report hearing English words. Whether Chinese-speaking people report Chinese words when listening to the illusion, or whether any reported words might be correlated with personality traits as previous investigations have demonstrated for listening to music in other cultures, is open to question. The present study aimed to address this. Methods A total of 308 right-handed, healthy volunteers (177 women and 131 men) were given the illusion test and asked to answer the Zuckerman-Kuhlman personality questionnaire (ZKPQ). Their depressive tendency was measured by the Plutchik-van Praag depression inventory (PVP). Results There was no gender effect regarding either the PVP score or the number of reported Chinese words from the illusion. Women scored higher on ZKPQ neuroticism-anxiety than men. The number of meaningful Chinese words reported was correlated with the ZKPQ impulsive sensation-seeking, aggression-hostility, and activity scores. Some words reported by participants who scored higher on these three traits were related in meaning to those scales. ConclusionOur preliminary results suggest that when Chinese-speaking people listen to the Deutsch "high-low" word illusion, they might use personality-related, specific cognitive schemata.
