Interactions between HIV and hepatitis B virus in homosexual men

Abstract: This study strengthens the evidence for a significant effect of HIV infection on the natural history of chronic HBV infection, which by prolonging the period of infectivity could have an important impact on the epidemiology of HBV infection in regions, or patient groups, with high HIV seroprevalence. There was no evidence of an important effect of HBV carriage on HIV disease progression.

“…HIV/HCV can be found in eight to 30% of all HIV infected individuals 3,5,28,29,31,32,35 , and approximately 50 -92% of these are intravenous drug users. Similarly, evidence of past HBV infection can be as high as 50 -80% of all HIV infected individuals 8,15,16,19 . This prevalence varies depending on the geographical area and the risk of the studied population 3,5,8,15,16,17,28,29,31,32,35 .…”

“…The course of chronic HBV and HIV co-infection will present with higher levels of HBV DNA and lower rates of clearance of the hepatitis Be antigen (HBeAg) [35]. HIV increases the risk of cirrhosis and end-stage liver disease in HBV coinfection [36].…”

“…6,7 Patients with chronic hepatitis B and underlying HIV infection (HIV-HBV co-infection) are also at higher risk of cirrhosis and hepatocellular carcinoma when compared with HBV-mono-infected individuals. 8 The ideal outcome of treatment for HBV would be to achieve HBsAg clearance with anti-HBs seroconversion, but this endpoint can be reached in only a few patients (fewer than 10% of HBV monoinfected patients have received interferon treatment, and probably even fewer HIV/HBV co-infected patients). A more realistic goal, therefore, is to efficiently and persistently suppress HBV replication and thus reduce the liver inflammation and scarring that can lead to liver cirrhosis and hepatocellular carcinoma.…”
